ROLE

Reporting to the Manager of Health Services, the Adult Wellness Coordinator supports Gwich’in Participants under the Gwich’in Comprehensive Land Claim Agreement (GCLCA) in strengthening overall wellness, life stability, and long-term healing. The role provides holistic, culturally grounded adult wellness support that extends beyond traditional treatment and aftercare, addressing substance use recovery, mental wellness, life skills development, and navigation of programs and services. Working directly with adults across the continuum of care—from crisis stabilization to long-term wellness planning, the coordinator delivers trauma-informed, culturally safe services rooted in Gwich’in values, land-based healing, and community connection. The position collaborates closely with territorial partners, community organizations, Elders, and cultural knowledge holders to ensure coordinated; wraparound supports that promote healthy, connected, and empowered lives for Gwich’in Participants.

RESPONSIBILITIES

  • Provide intake, assessments, and wellness planning for adults seeking support for mental health, addictions, grief, trauma, and general life stability.
  • Create individualized wellness plans that include cultural, emotional, physical, mental, spiritual, and social supports.
  • Support adults in navigating GNWT, federal, non-profit, and community programs.
  • Support adults seeking detox, treatment, or recovery programs across NWT and Canada.
  • Maintain aftercare plans and support clients with post-treatment reintegration.
  • Maintain a directory of territorial, national, and land-based treatment programs.
  • Facilitate connections to cultural counsellors, Elders, knowledge holders, and On-the-Land programs.
  • Participate in healing gatherings, seasonal camps, and cultural programming.
  • Encourage, incorporate, and document cultural goals in all wellness plans.
  • Assist adults in accessing housing supports, justice diversion programs, income support, medical services, and employment/education resources.
  • Build strong partnerships with health, justice, housing, education, and social services.
  • Deliver education and awareness initiatives related to mental health, addictions, and life skills.
  • Promote harm reduction and safe practices when appropriate.
  • Maintain confidential client files using GTC-approved systems.
  • Track and report program outcomes.
  • Contribute to strategic planning, program evaluations, and department development.
  • Attend ongoing training in trauma-informed practice, Indigenous wellness approaches, addictions, cultural safety, harm reduction, and community health.
  • Additional duties as assigned

QUALIFICATIONS

  • Diploma or degree in Social Work, Mental Health & Addictions, Counselling, Indigenous Wellness, or a related human services field.
  • Equivalencies with relevant training and lived experience will be considered.
  • Minimum 2 years of experience supporting adults in mental health, wellness, addictions, or community support roles.
  • Strong understanding of trauma-informed, harm reduction, and culturally safe practices.
  • Knowledge of adult learning, life skills development, and crisis intervention.
  • Experience supporting Indigenous adults; knowledge of Gwich’in culture, language, values, and land-based practices is a strong asset.
  • Ability to collaborate with territorial systems (health, housing, justice, education, etc.).
  • Strong communication, relationship-building, and organizational skills.
  • Proficiency with MS Office (Word, Excel, Outlook, PowerPoint).
  • Ability to maintain confidentiality with sensitive client information.
  • Valid Class 5 Driver’s License and ability to travel.
