Position Title: Health and Wellness Manager – Lake Bonavista Village

(Temporary 12-18 months)

As the Health and Wellness Manager, you are responsible for working in conjunction with other United teams by role modelling and providing leadership to the Health and Wellness team. Reporting directly to the Executive Director the primary responsibility of the Health and Wellness Manager is to ensure the well-being of the residents through planning and delivery of high-quality care services consistent with the policies, procedures and philosophy of United Active Living.

Responsibilities (includes but not limited to):

  • Completes move in assessments, and participates in follow up assessments with the interdisciplinary team (United Minds, United Cares, Home Care, physician, pharmacy, family, other community members etc.) as indicated
  • Identifies those residents who require family participation in the assessment process to ensure accuracy.
  • Develops wellness plans with residents and their families including both short- and long-term goals. Updates team members of wellness plan goals to enhance awareness.
  • Communicates with residents and/or their families regarding updates, concerns, and/or residents’ successes.
  • Monitors to ensure scheduled assessments are completed when required.
  • Reports to the Executive Director, escalated concerns from residents, families, and or team members to resolve concerns.
  • Collaboration with family physician and pharmacy as needed to support residents care needs.
  • Participates in the clinical on-call rotation.
  • Participates in the workplace safety program.
  • Responds to and acts professionally in emergency or disaster situations
  • Leadership and Supervision
  • Demonstrates an appreciation of the heritage, values and wisdom of the residents and an understanding of the aging process, and/or living with a disability.
  • Provides leadership and supervision to Health and Wellness team. Including recruitment, coaching, performance management and training.
  • Assumes responsibility for staff scheduling.
  • Works collaboratively with peers and other team members.
  • Uses professional and respectful communication skills at all times, including potentially sensitive or emotionally charged situations.
  • Demonstrates a commitment to good customer service.

Other Duties

  • Participates in in-service education or committees as required.
  • Participates in and supports the resident-centered activity programs, including creative expression activities.
  • Assists with quality enhancement activities.
  • Shares in the responsibility for providing and maintaining an environment that is conducive to the safety and physical and emotional well-being of residents and personnel.
  • Performs other duties as assigned.

Qualifications:

  • Post secondary education in Nursing with minimum of LPN program of studies with current license with the College of Licensed Practical Nurses of Alberta;
  • Must have current CPR for Health care professionals;
  • Proficient with Microsoft Office;
  • Experience working with older adults with dementia an asset;
  • 2-3 years in a supervisory role preferred;
  • Must be able to read, write and speak fluently in the English language.

Physical Requirements:

  • Ability to lift up to 25 pounds;
  • Hours of work Days however there may be require some evening or weekends as needed for supervision of the team
  • May require being on your feet for extended periods of time.
