About the Job
Reporting to the Director of Clinical Mental Health and Wellness, the Manager of Assertive Community Treatment Team (ACTT) provides operational, and clinical leadership to the Assertive Community Treatment Teams (ACTT). The Manager is responsible for ensuring that services are delivered in accordance with the Ontario Association for ACT & FACT (OAAF) ACT Program Standards, organizational priorities, accreditation requirements, legislative requirements, and evidence-informed best practices.

The Manager leads two interdisciplinary teams comprised of Psychiatry, Nursing, Social Work, Occupational Therapy, Addiction Specialists, Vocational Specialists, Peer Support Workers, Mental Health Clinicians, Supervisor, and Program Administration who provide intensive, community-based treatment, rehabilitation, and recovery-oriented services for adults living with severe and persistent mental illness and concurrent disorders.

The Manager fosters a culture of recovery, equity, inclusion, quality improvement, innovation, and clinical excellence while ensuring high fidelity to the ACT model. The role provides clinical oversight, operational leadership, financial stewardship, staff development, risk management, and system collaboration to achieve positive client outcomes and organizational objectives.

Duties and Responsibilities

Clinical Leadership
  • Provides overall clinical leadership and operational management of interdisciplinary ACT Teams.
  • Ensures services are delivered in accordance with the current OAAF ACT Program Standards and maintains high fidelity to the ACT model.
  • Promotes recovery-oriented, trauma-informed, culturally responsive, anti-racist, and person-centred care.
  • Oversees clinical decision-making, client flow, admissions, service transitions, discharge planning, crisis response, and risk management.
  • Provides advanced clinical consultation for complex client situations.
  • Ensures individualized recovery planning reflects client goals, strengths, and preferences.
  • Supports timely access to ACT services through effective intake, referral, and triage processes.
  • Ensures teams provide flexible community-based services, including maintaining required community contact expectations and responsiveness to client need.

Program Operations
  • Manages day-to-day operations of both ACT Teams.
  • Leads daily interdisciplinary clinical meetings to coordinate treatment planning, risk management, and service delivery.
  • Ensures staffing models support seven-day service delivery and 24-hour crisis response consistent with ACT standards.
  • Oversees scheduling, workload allocation, and resource utilization.
  • Monitors program capacity, waitlists, and client flow while maintaining fidelity requirements.
  • Ensures organizational policies, procedures, and clinical standards are implemented consistently.
  • Leads program planning, implementation, and evaluation of new initiatives.

Quality, Fidelity and Performance Improvement
  • Leads continuous quality improvement initiatives using data to improve clinical outcomes and service effectiveness.
  • Monitors program performance indicators, including service intensity, community contacts, hospitalization rates, documentation quality, and client outcomes.
  • Coordinates internal and external ACT fidelity reviews and develops action plans to address recommendations.
  • Ensures compliance with Ontario Health reporting requirements, accreditation standards, and organizational quality initiatives.
  • Promotes evidence-informed practice and implementation of emerging best practices.

Staff Leadership
  • Recruits, hires, mentors, coaches, develops, and evaluates interdisciplinary staff.
  • Provides regular individual clinical supervision.
  • Conducts in vivo coaching, observation, and side-by-side supervision in community settings.
  • Supports professional development and succession planning.
  • Promotes staff wellness, psychological safety, and employee engagement.
  • Manages performance concerns through coaching, progressive discipline, and performance management processes where required.
  • Fosters an inclusive, collaborative, and psychologically safe team culture.

Clinical Governance
  • Ensures compliance with professional standards, privacy legislation, documentation standards, and ethical practice.
  • Oversees quality and timeliness of clinical documentation, including assessments, recovery plans, OCAN completion, progress notes, and risk assessments.
  • Ensures informed consent, substitute decision-making processes and client rights are upheld.
  • Oversees incident management, critical incident reviews and implementation of corrective actions.
  • Promotes client safety and organizational risk management.

Financial and Resource Management
  • Develops, monitors, and manages program budgets.
  • Ensures appropriate allocation of financial and human resources.
  • Supports business planning, funding proposals, and strategic initiatives.
  • Identifies opportunities for program growth and service innovation.

Community and System Leadership
  • Builds collaborative partnerships with hospitals, primary care, Ontario Health Teams, community mental health providers, housing providers, justice partners, emergency services, and other community organizations.
  • Represents SHIP on regional committees, planning tables, and system initiatives.
  • Supports seamless transitions across the continuum of care, including ACT-to-ACT transfers and step-down planning.
  • Advocates for equitable access to community mental health services.

Administrative Responsibilities
  • Maintains accurate statistical reporting and program performance data.
  • Prepares operational reports and quality improvement reports.
  • Participates in strategic planning and organizational committees.
  • Supports policy development and program evaluation.
  • Performs other duties consistent with the role.

Qualifications
  • Master's Degree in Social Work, Nursing, Occupational Therapy, Psychology, or another regulated health profession.
  • Current registration in good standing with the applicable regulatory college.
  • Minimum five years of progressive leadership experience in community mental health.
  • Demonstrated experience leading interdisciplinary clinical teams.
  • Significant knowledge of Assertive Community Treatment philosophy and OAAF ACT Program Standards.
  • Extensive knowledge of recovery-oriented practice, psychosocial rehabilitation, trauma-informed care, and concurrent disorders.
  • Experience managing complex clinical risk and crisis intervention.
  • Demonstrated knowledge of quality improvement methodologies and program evaluation.
  • Strong knowledge of the Mental Health Act, Health Care Consent Act, PHIPA, Substitute Decisions Act, and other relevant legislation.
  • Exceptional leadership, coaching, conflict resolution, and change management skills.
  • Excellent communication, organizational, and relationship-building abilities.
  • Valid Ontario Class G Driver's License and access to a reliable vehicle with appropriate insurance.

Reporting Function

The Manager will report to the Director of Clinical Mental Health and Wellness. They will be responsible for overseeing the Assertive Community Treatment Teams and they will have the authority to hire/fire/discipline and manage these staff members appropriately. They will work with both internal and external individuals and will interact with all levels within the organization.

They will be a support system, resource, and an advocator for the team and organization.

Working Conditions and Physical Requirements
  • Monday to Friday, with flexibility to meet operational requirements.
  • Participation in an on-call leadership rotation.
  • Frequent community travel by vehicle to.
  • Combination of office, community, and virtual work environments.
  • Occasional evening and weekend work to support program operations.
  • Regular use of electronic health records and computer systems.
  • Able to work well with internal and external clients
  • Able to drive, walk (site locations) regularly
  • Able to sit for prolonged periods of time
  • Able to answer and communicate telephone calls and general computer use
  • Computer screen use (multiple screens if applicable) for prolonged periods of time
  • Able to lift up to 20lbs (product, documents, etc.)
