Full job description

Position Summary:
We are seeking a skilled Support Worker to provide one-to-one support for individuals in their family homes. Responsibilities include assisting with personal care, medication administration, seizure monitoring, stretches, recreation, and other meaningful daily activities. This role requires flexibility, creativity, and strong time management skills.

This is a family-directed position. The caregiver, along with a PHSS Coordinator, will support you through training, policy review, and provide ongoing guidance. This role is ideal for someone who works well independently, takes initiative, and values building meaningful connections.

Applicants with DSW, PSW, or minimum one year of Nursing education will be given preference.

Responsibilities:
Assist with personal care in a respectful and dignified manner

Provide support in family homes and follow direction from families and the person supported while working independently

Support community participation and plan meaningful activities and skill-building opportunities

Assist with mental health support, behaviour redirection, and de-escalation as needed

Remain calm in stressful situations and respond professionally to risk behaviours

Follow support strategies and safety protocols

Work collaboratively with families with different perspectives and approaches

Document incidents and report to a supervisor as required

Participate in ongoing and annual training

Requirements:
DSW, PSW, or Nursing education

Training or experience supporting individuals with verbally and physically aggressive behaviour, or willingness to train

Experience supporting individuals with Autism and Behaviour Support Plans, or willingness to train

Reliable, punctual, confident, and able to maintain confidentiality

Valid G driver’s license

Current First Aid & CPR certification and Vulnerable Sector Check

CPI and/or SMG certification, or willingness to train

Proof of COVID-19 vaccination (minimum one dose of a Public Health Ontario approved vaccine)

Willingness to support individuals in the community for 3+ hours

Weekend availability and weekday flexibility required
