Full job description

Durham Care Clinic + Pharmacy is hiring a Clinical Screening Associate to review patient charts in our EMR, identify patients with care gaps, and flag them to our physicians and pharmacists for action. Designed for an internationally trained pharmacist or physician who is not yet licensed in Ontario.

About the role

You will be the clinical eyes on our patient panel between visits. Working inside the EMR, you review charts systematically, apply clinical criteria to labs, diagnoses and medication lists, and build clean, actionable lists of patients who need something: a vaccine, a dose adjustment, a missing guideline-directed therapy, an overdue test. Our physicians and pharmacists then act on what you find.

This is a non-licensed, chart-review role. It suits an internationally trained pharmacist or physician who has strong medical and drug knowledge, reads labs fluently, and wants Canadian clinical experience inside a cardiometabolic practice while working toward licensure. You will not see patients, prescribe, or make treatment decisions; every flag you raise goes to a licensed clinician for review.

What you will do

  • Review patient charts in the EMR against screening criteria set by our physicians and pharmacists, working through the panel by condition, by physician, or by recall list.
  • Read and interpret labs (A1C, lipids, eGFR and ACR, liver enzymes, electrolytes, TSH, B12, iron studies) and identify results that are out of target, trending the wrong way, or overdue.
  • Reconcile medication lists against diagnoses to spot missing therapies, duplications, interactions, renal dose issues, and medications without an indication.
  • Check immunization status against Ontario's publicly funded schedule and flag patients due for vaccines.
  • Document each finding in the EMR in a standard format: patient, the gap, the supporting evidence, and the suggested action for the clinician to consider.
  • Build and maintain recall lists and queues so physicians and pharmacists can work through flagged patients efficiently.
  • Track what happens to each flag (actioned, deferred, not applicable) and report screening volumes and outcomes monthly.
  • Help refine the screening criteria as guidelines change, and support quality-improvement and sponsored screening programs run by the clinic.

Must-have qualifications

  • A pharmacy degree (BScPhm, PharmD or equivalent) or a medical degree (MBBS, MD or equivalent) from outside Canada, with at least two years of clinical practice in your home jurisdiction.
  • Strong working knowledge of medicine and pharmacology across diabetes, cardiovascular, renal and metabolic disease, including current Canadian or international guideline targets.
  • Confident interpretation of common laboratory results and trends.
  • Hands-on experience working in an electronic medical record, and the ability to navigate charts, labs, medication lists and recall tools quickly. Experience with a Canadian EMR (Accuro, PS Suite, OSCAR, TELUS Collaborative Health Record) is an asset.
  • Methodical and accurate: you can review a high volume of charts to a consistent standard and document findings clearly.
  • Clear written English; able to summarize a clinical picture in two or three lines a busy clinician can act on.
  • Comfortable with Microsoft Excel or Google Sheets for lists and tracking.
  • Eligible to work in Canada.
