Job Overview

We are seeking a dedicated and detail-oriented Clinical Pharmacist to join our healthcare team. The ideal candidate will provide expert medication management and patient care within a hospital setting, ensuring safe and effective use of pharmaceuticals. This role offers an opportunity to work in a dynamic environment, collaborating with healthcare professionals to improve patient outcomes. The Clinical Pharmacist will be responsible for reviewing prescriptions, counseling patients, and supporting clinical decision-making processes. Candidates with hospital experience, especially in acute care or pediatrics, are highly preferred.

Responsibilities

  • Review and interpret physician prescriptions to ensure accuracy and appropriateness of medication therapy
  • Provide expert consultation on medication selection, dosage, interactions, and side effects
  • Counsel patients on proper medication usage, potential side effects, and safety precautions
  • Collaborate with healthcare teams to develop and implement individualized patient care plans
  • Administer medications as prescribed in accordance with hospital policies and medical terminology standards
  • Monitor patient responses to medications and adjust therapy as needed for optimal outcomes
  • Maintain accurate documentation of medication administration and patient interactions
  • Stay current with new pharmaceutical developments, guidelines, and best practices in patient care
  • Experience in sterile and non-sterile compounding is preferred
  • Experience in homecare/home infusion processes is preferred

Requirements

  • Valid license as a licensed Pharmacist with relevant state certification
  • Hospital experience is preferred; acute care experience is highly desirable
  • Knowledge of pediatrics and specialized medication protocols for children is a plus
  • Strong understanding of medical terminology and medication administration procedures
  • Excellent communication skills for effective patient counseling and interdisciplinary collaboration
  • Ability to work in fast-paced environments while maintaining attention to detail
  • Commitment to providing high-quality patient care with a focus on safety and efficacy

Pay: $43.33-$64.91 per hour
