Full job description
Jasper Place Wellness Centre’s (JPWC) Bridge Healing provides short-term housing for individuals who have been discharged from medical care. While here, JPWC Bridge Healing team helps individuals work towards housing and plan for the future with stability and dignity. Reporting to the Site Team Lead, the Health Care Aide (HCA) provides person-centered support within Transitional Housing while working collaboratively with the Jasper Place Wellness Center Clinic.
RESPONSIBILITY
- · Implement and monitor individualized care plans under the supervision of Bridge Healing leadership.
- · Provide personal care and support with activities of daily living in a manner that promotes dignity, independence, and guest choice.
- · Reinforce established care plans, health education, and wellness strategies within the Health Care Aide scope of practice.
- · Assist residents with meals, laundry, housekeeping, and other essential daily living needs.
- · Observe, document, and promptly report changes in guest health status, behavior, or well-being.
- · Conduct resident wellness checks and support ongoing health monitoring activities.
- · Facilitate resident access to Community Health Centre, community supports, and external resources.
- · Support the coordination of appointments, transportation, and continuity of care.
- · Promote guest autonomy, recovery, and well-being through person-centered, trauma-informed approaches.
- · Ensure guest’s discharge instructions and continued recovery plan are communicated to all necessary parties.
- · Participate in interdisciplinary guest care planning and goal setting.
- · Maintain accurate, timely, and confidential documentation in accordance with organizational policies and procedures.
- · Adhere to infection prevention and control practices to maintain a safe environment for guests, staff, and visitors.
- · Respond appropriately to emergencies, critical incidents, and guest concerns within the scope of practice and organizational protocols.
QUALIFICATIONS
- Recognized Health Care Aide Certificate or equivalent
- Grade 12 or GED CPR/BCLS Registration with the Alberta Health Care Aide Directory
- A clear criminal record check with vulnerable sector screening
- Food Safe Basic or equivalent
- Able to communicate information in English both verbally and in writing
- Strong interpersonal skills with the ability to deal courteously and positively with residents/clients.
- Physically and mentally fit to perform the duties of the position (may be required to lift, move, push, or pull objects weighing up to 50 pounds)
- Ability to work on his/her feet for a complete shift while constantly bending, stretching, walking, lifting, or moving things.
- Experience working with Indigenous peoples and individuals from diverse cultures Experience working with marginalized communities
- Experience working in a Harm-Reduction Framework and Setting
- Experience using Naloxone and Responding to Overdoses
