Peer Support Worker – Hart Hub

1 Temporary Full-time – contract until August 2027

The Peer Support Worker will provide person‑centered, recovery‑oriented supports within the HART Hub and the broader community of Lambton County, drawing on their lived experience to build trust, foster connection, and promote wellness among day-program participants. This worker will be stationed at the HART Hub and meet clients within the community as needed (where engagement at HART may be a barrier), accompanied by the County of Lambton’s Community Outreach Team or a CMHA worker. They may also collaborate with and transition appropriate clients to services at the Lambton Mental Wellness Centre.


Reporting to the Manager of Housing and Client Services, the incumbent will be responsible for the following functions:

  • Engaging clients through peer‑based rapport building, offering active listening and empathetic conversation.
  • Providing practical support, including problem-solving, developing life skills, and navigating community resources and systems.
  • Facilitating group or one‑to‑one wellness activities, such as meaningful activity engagement, goal setting, and skill‑
  • Supporting system navigation, helping participants connect with housing, healthcare, income supports, and other community services.
  • Referring to appropriate services for additional supports, including external partners.
  • Collaborating with HART staff to help maintain a welcoming and safe environment for all.
  • Documenting services and outcomes in accordance with program requirements.

Qualifications:

  • Post-secondary diploma in a relevant discipline such as a Social Service Worker, or Addictions and Mental Health with a Professional Registration;
  • 1-2 years’ experience in a health related organization and experience working with individuals with mental health issues;
  • Personal lived experience in recovery from a mental health and/or addiction challenges;
  • A valid Ontario Driver’s Licence and access to a vehicle.

Hours of Work: 35 hours/week. Flexible hours are required to meet the position requirements.


Salary:
Commensurate with experience and education within the range: $27.64-$33.63 inclusive of HOOPP.
