Position Summary:

Reporting to the Manager, Surgical Services the successful candidate is responsible for guiding patients through the surgical process, acting as a point of contact for patients, families, and the healthcare team. Coordinating care, facilitate communication, and ensure patients receive timely and appropriate treatment. Their responsibilities span from pre-operative planning to post-operative care, encompassing patient education, advocacy, and resource navigation. The successful candidate will participate in quality-based research and lead implementations of quality initiatives. The successful candidate will work collaboratively with the multidisciplinary team to prioritize work and act as the point of contact for the team.

There is a specific focus on our Orthopedic and complex surgical patient populations, in addition to supporting all surgical specialties.

Responsibilities:

  • The navigator will provide support to patients and families as required
  • The navigator will follow up with patients referred by the care team with complex needs and post operative patients that have deviated from the care pathway and require supports.
  • The navigator will lead follow up with patients, when required, related to bundled care post op Physiotherapy and bundled care reconciliation.
  • The navigator will lead case conferences when required.
  • The navigator will lead our Total Joint Education Class and lead any updates based on best practices and evidence.
  • The navigator will lead the development of supporting documents related to surgical pathways and post operative instructions (care pathways, order sets, etc).
  • The navigator willl act as a liasion for surgeons and their support staff.
  • The navigator will ensure that Patient Reported Outcome Measures (PROMS) are compeleted on all relevant patients and oversee our hospital process.
  • The navigator will be required to evaluate program deliverables, and analyze performance data to meet and support varies quality improvement projects
  • Lead quality-based research and bring new initiatives to the leadership team for review.
  • Continue evaluating existing programs and resources; acting as a support to our Perioperative, Preoperative, and Post operative areas.
  • The navigator role will lead the ongoing improvements on orthopedic and other surgical specialties related education material including innovation regarding dissemination of this material.
  • The navigator will participate in Quality initiatives and Quality Councils when required.
  • Other duties assigned.

Qualifications:

  • Bachelor's degree required (in a health related discipline)
  • Five years recent clinical experience in an acute care setting preference given to orthopedic/surgical experience and/or in a formal Quality Improvement Role and/or active involvement in leading Quality Improvement initiatives at the corporate level.
  • Membership in the Registered Nurses Association of Ontario, preferred.
  • CNA certification in Orthopaedics preferred.
  • Experience in clinical research and formal teaching experience.
  • High level of initiative, works independently and able to effectively lead project teams to achieve outcomes.
  • Experience with Quality Based Procedures (QBP).
  • Improvement Advisor certification through the Institute for Healthcare Improvement (IHI), certification in Lean and/or related quality improvement methodology (eg. Six Sigma) Project Management certificate an asset.
  • Current registration with a health professional regulatory body and/or be a member in good standing with a professional association preferred.
  • Demonstrated knowledge and experience with chart audits, performance measurement, data reporting and analysis.
  • Evidence of effective customer service, organizational, interpersonal, communication, leadership, judgment, critical thinking and decision-making skills.
  • Computer skills in MS Word and PowerPoint, Email, Internet required.
  • Proficiency in Microsoft Office Excel and Access is an asset.
  • Ability to work effectively within a multi-disciplinary team and with a variety of levels of staff across departments and the organization.
  • Flexible self-directed and able to work effectively with minimal supervision.
  • Consistent and regular attendance required.
