Nurse Practitioner Job Description

Village Family Health Team serves the rapidly growing neighbourhoods of Liberty Village, King/Queen West and Parkdale in Southwest Toronto. Position Summary:

The Nurse Practitioner (NP) plays an integral role in the care of the patients of Village Family Health Team (FHT). The NP will support patient care and the delivery of FHT programs working collaboratively within an inter-professional healthcare team. The NP will practice within their full scope of practice as outlined in the Standards of Practice by the College of Nurses of Ontario. The NP role has three major elements:

  • Chronic disease management
  • Same day urgent care
  • Responsible for a roster of patients

Key Responsibilities:

  • Conduct full patient assessment and prepare a plan for intervention and/or for referral to FHT programs as appropriate
  • Identify, analyze and provide appropriate treatment and/or referral for patient’s physical and psychosocial health care problems
  • Implement various goal-oriented, time-limited intervention plans, including education in collaboration with patient and/or family and team
  • Evaluate and document outcomes of individual patient care
  • Participate as a clinical team member in the delivery of targeted programs
  • Provide timely and informative reports as directed and requested by the Executive Director and/or Physician Lead.
  • Participate in team building, program development, and quality improvement activities.
  • Participate in weekly staff meetings
  • Provide service in after-hours (evening and Saturday) clinic, as required
  • Identify gaps and needed modifications in primary care services

Qualifications:

  • A Bachelor’s of Science in Nursing with a Primary Health Care Nurse Practitioner Certificate§ A Master’s Degree is considered an asset
  • Current registration with the College of Nurses of Ontario as an RN(EC) required
  • Current Membership with RNAO and evidence of professional liability insurance
  • Work effectively, both independently and as part of an interprofessional team
  • Flexibility and ability to adapt to change
  • Ability to utilize an electronic documentation of health records using a Electronic medical Record system (Accuro is the EMR at Village FHT)
  • Ability to work flexible hours (some evening work is required)

Reporting Structure:

The Nurse Practitioner will report to the Executive Director for day-to-day operations.

Job Specifications:

Location: 171 East Liberty St., Unit 102.

Hours of Work: 37.5 hours per week, generally aligning with FHT practice operations, with some evening and Saturday clinics.

Status: 1.0 FTE, full-time, permanent.
