Community Nurse Manager (Branch Lead)

Lead People. Build Trust. Shape the Future of Community Care.

Toronto, ON | Full-Time | Registered Nurse (RN) or Registered Practical Nurse (RPN)

Ready for Your Next Chapter?

You've built a strong clinical foundation. You make sound clinical decisions, earn the trust of clients and families, and enjoy helping others succeed.

Now you're ready for something more.

At CareHop, we're looking for an exceptional Registered Nurse (RN) or Registered Practical Nurse (RPN) to grow into a leadership role where you'll mentor employees, guide families through complex healthcare decisions, build trusted community partnerships, and help shape the future of community care.

You'll work directly with the CEO and senior leadership team to help build one of Ontario's most innovative home and community healthcare organizations.

This isn't just your next job—it's your opportunity to become a healthcare leader.

Why CareHop?

Happier People. Better Care.

At CareHop, we believe happier employees provide better care. Better care builds trust, and trust creates lasting relationships with clients, families, employees, and community partners.

We don't simply provide healthcare services—we help people navigate life's healthcare challenges by bringing together the right combination of clinical expertise, family support, community resources, publicly funded healthcare, technology, and CareHop services.

As a Community Nurse Manager, you'll help families identify the next meaningful step toward safer, healthier, and more independent living.

We're also proud of the workplace we've built. CareHop has earned an 88 Work Wellbeing Score on Indeed, with employees consistently recognizing our supportive leadership, collaborative culture, meaningful work, and opportunities for growth.

Compensation & Benefits

  • Competitive compensation: $78,000–$92,000 per year, based on experience and qualifications
  • Performance bonus
  • Comprehensive Extended Health & Dental Benefits
  • Paid vacation
  • Leadership development and mentorship from senior leaders
  • Career advancement opportunities as CareHop continues to grow

What You'll Do

As our Community Nurse Manager (Branch Lead), you will:

  • Lead, coach, and inspire a team of nurses and personal support workers.
  • Conduct client consultations, nursing assessments/reassessments, supervisory visits, and quality reviews.
  • Guide families through complex healthcare decisions by understanding their goals, challenges, and existing supports, then developing thoughtful, person-centred care plans.
  • Build trusted relationships with clients, families, hospitals, physicians, discharge planners and community partners.
  • Foster employee engagement, accountability, and a positive team culture.
  • Drive quality improvement initiatives and support branch performance.
  • Develop, review and update individualized care plans.
  • Medication management, including Best Possible Medication History (BPMH) and medication reconciliation
  • Review remote monitoring alerts and act as needed
  • Provide direct client care when needed to support your team and ensure continuity of care.
  • Identify opportunities to improve client outcomes, strengthen partnerships, and support the continued growth of the branch.

Who Thrives in This Role?

We're looking for someone who:

  • Is a Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.
  • Has developed strong clinical judgment and critical thinking through hands-on nursing experience.
  • Naturally builds trust and meaningful relationships.
  • Has at least three years of nursing experience and is ready to take the next step in the leadership journey.
  • Enjoys mentoring and developing others.
  • Thinks beyond the immediate problem and seeks practical, long-term solutions.
  • Is proactive, curious, and excited by new challenges.
  • Leads with compassion, integrity, and accountability.
  • Holds a valid Ontario driver's licence and has access to a reliable vehicle.

Whether you're an experienced RPN ready to take the next step in your leadership journey or an RN looking to make a broader impact in community healthcare, we'd love to hear from you.

Leadership or home care experience is an asset—but leadership potential, relationship-building skills, and a passion for making a difference matter just as much.

Why This Role Is Different

This isn't about managing schedules.

It's about building a stronger healthcare system—one family, one employee, and one partnership at a time. If you're looking for a place where your ideas are valued, your leadership is developed, and your impact grows alongside the organization, you'll feel at home at CareHop.

You'll have the opportunity to:

  • Develop and mentor future healthcare leaders.
  • Build meaningful partnerships with hospitals, physicians, and community organizations.
  • Influence innovative programs and new models of care.
  • Help shape the future direction of CareHop.
  • Grow alongside an organization committed to innovation, continuous improvement, and exceptional care.
