- Coordinate care across providers, schools, community supports, and regional health services as needed.
- Support physicians and nurse practitioners in the program with transition of adult patients from other program sites for continuity of care into the community-based clinic as needed.
- Support patient engagement with other community-based chronic pain programs, and liaise with other program contacts as needed (ex. St. Joseph’s Care Group or Interventional Pain Service at TBRHSC)
- Assist with procedures or treatments (e.g., injection preparation, post-procedural monitoring, documentation).
- Contribute to the development and delivery of group programs, workshops, and resource materials.
- Participate in quality improvement, program evaluation, and data collection activities.
- Ensure compliance with documentation standards, infection control, and privacy regulations.
Required Qualifications
- Current registration with the College of Nurses of Ontario as a Registered Nurse.
- Minimum 2 years of clinical nursing experience, preferably in pediatrics, mental health, community care, chronic disease management, and/or research activities.
- Demonstrated ability to work collaboratively in an interdisciplinary team setting.
- Strong communication, organizational, and documentation skills.
- Commitment to patient- and family-centered care.
Preferred Qualifications
- Experience working with children, youth, adults and families living with chronic pain or mental health conditions.
