  • Employment Status: Full Time - Permanent
  • Full time Hour: 37.5 per week

Quest Community Health Centre (Quest CHC) is a dynamic non-profit, interdisciplinary health care organization providing holistic primary health care, health promotion, and community services throughout Niagara. Our client-centred approach addresses social determinants of health and works to reduce barriers to quality car

The Nurse Practitioner provides comprehensive primary health care services across Quest CHC and community settings, with a focus on health promotion, disease prevention, harm reduction, and trauma-informed care. Working within an interdisciplinary team, the NP provides assessment, diagnosis, treatment, health education, referrals, and ongoing care while supporting clients with complex health and social needs. The role also contributes to community partnerships, care planning, student supervision, health initiatives, and continuous improvement of primary care services.

QUALIFICATIONS INCLUDE:

  • A Master’s degree in Nursing required, or a combination of education and experience as sanctioned by the College of Nurses of Ontario.
  • Current and valid registration in the Extended Class with the College of Nurses of Ontario and current registration in good standing with the College of Nurses of Ontario.
