Nurse Practitioner, Temp Full Time 1 Year Contract with the Opportunity for Extension
Location: Virtual (primarily) with some in-person coverage in Neyaashiinigmiing, Ontario
Compensation: $124,478 plus benefits based on a 35 hours/week (Monday to Friday)

Noongwa Wellness is a newly established Indigenous Primary Health Care organization rooted in community, culture, and wholistic wellness.
We are seeking a Nurse Practitioner to join our team to provide exceptional virtual clinical care to support the delivery of high quality culturally safe, team based person-centred care. This position is ideal for someone energized by clinic start-up, relational care, and working collaboratively across organizations. This role combines clinical expertise with leadership in shaping workflows, fostering collaboration, and advancing Indigenous models of self-determined care.
PURPOSE AND SCOPE
As a key member of Noongwa Wellness, the Nurse Practitioner (NP) is an integral part of an interdisciplinary health care team providing exceptional team-based care to clients.
This NP role supports the following core responsibilities:
- Increasing access to primary health care through virtual and in person visits
- Patient outreach and engagement
- Patient onboarding and supported attachment.
The NP ensures patients of Noongwa Wellness receive timely access to primary care, supports continuity of care, and strengthens patient-provider relationships.
DUTIES AND RESPONSIBILITIES
Supported Attachment
- Facilitate onboarding of patients to primary care, ensuring a smooth transition for long-term attachment
- Conduct initial virtual assessments, health history intake, and risk screening
- Provide guidance and education on navigating the healthcare system and accessing services.
- Collaborate with care teams to ensure timely follow-up and continuity of care
- Maintain accurate documentation in EMRs for all onboarding activities and clinical encounters.
Virtual NP
- Conduct virtual care including medication renewals, episodic appointments, and urgent results follow-up
- Complete virtual documentation, including forms, genograms, risk assessments, and EMR charting
- Monitor patients with multiple comorbidities through chart reviews and coordination with the care team.
Referral & Preventative Care Management
- Conduct virtual periodic health reviews for preventative screening
- Update EMR with follow-up dates, screening intervals, and referral details
- Document referral pathways including specialist/agency names, follow-up timelines, and outcomes.
Chronic Disease Management
- Provide ongoing monitoring and support for patients with chronic conditions
- Collaborate with care teams to maintain individualized care plans and chronic disease management strategies.
Quality Improvement & Best Practices
- Participate in the development, review, and implementation of clinical policies and directives
- Contribute to education, training, and dissemination of best practices across sites
- Support continuous improvement initiatives to enhance care delivery and patient outcomes.
- Additional duties as appropriate in a growing wholistic primary health care team
REQUIREMENTS
- Master in Nursing Degree with valid Nurse Practitioner certification, preferably in Primary Health Care
- Bachelor of Science in Nursing degree
- Current registration in good standing as a Nurse Practitioner with the College of Nurses of Ontario (CNO)
- Minimum 2 years’ experience in community health or primary care preferred
- Experience providing virtual care considered a strong asset
- Demonstrated ability to work independently and collaboratively in an interprofessional, remote, and in-person environment
- Obtain a satisfactory police check as a condition of employment
- Up-to-date immunizations and records
- Knowledge of Anishinaabe health issues and commitment to culturally safe care is considered a strong asset.

WORK ENVIRONMENT
- Primarily remote for virtual coverage
- In-person presence as required
- Some evening coverage to increase accessibility.
ORGANIZATIONAL RELATIONSHIPS
- Reports directly to the Executive Director and Physician Dyad
- Respects formal and informal protocols of the Chippewas of Nawash Unceded First Nation Community.
PHYSICAL DEMANDS AND WORK ENVIRONMENT
- Operates in a remote office
- Frequent use of computers and occasional travel
- Exposure to moderate stress due to competing demands
- Occasional exposure to smoke from traditional practices (e.g., tobacco, sage, sweetgrass, cedar).
