Position: Mental Health First Response Worker – HART Hub/Mobilecare


Program: Sarnia Site

Position Status: 1 Permanent Full-time


QUALIFICATIONS:
Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the CNO. (Preference to those with Canadian Certification in Psychiatric/Mental Health Nursing); Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or Bachelor’s Degree with an applicable college diploma in Addictions and current registration with the C.A.C.C.F


SALARY RANGE:
As per salary grid

HOURS OF WORK: 35 hours per week

STARTING DATE: To be determined


This position is responsible for providing Crisis supports to both Mobilecare, HART Hub and other various locations that CMHA serves. The incumbent will gather collateral information from primary care providers, psychiatrist, family or care givers, and in coordination with the multidisciplinary team, assist in the development of recommendations and treatment planning. Brief services will be offered to individuals who require supports, within areas that have been identified through assessment.


Reporting to the Integrated Manager of Client Services, the incumbent will be responsible for the following functions:

  • Complete detailed standardized inter-RAI ESP assessment to determine client needs and services
  • Providing intervention and brief solution focused support to individuals experiencing a psychiatric crisis (intervention may be via telephone or by personal contact);
  • Providing a quick response to urgent situations that arise;
  • Providing client needs identification through on-going assessment and team oriented treatment planning.
  • Implementing treatment plans that focus on strengths, wellness and empowerment to support recovery and goal achievement.
  • Providing crisis prevention and intervention planning with the intention of preventing a relapse and/or hospitalization.
  • Routinely assesses client’s physical and mental health and liaises with appropriate physicians/psychiatrist regarding identified concerns.
  • Acting as a resource person or an advocate to assist individuals in obtaining information/accessing services/supports which are deemed needed
