Full job description

Summary of Duties

Provides direct nursing care for residents of Trinity Manor in order to encourage optimum level of functioning. Supports residents in meeting their physical, social, emotional, and spiritual needs.

qualifications

Current LPN license, valid in Saskatchewan. Current First Aid / CPR certification. Must provide original copy of Criminal Record Check for vulnerable sector.

Knowledge. Skills, and Abilities

  • Good interpersonal, organizational, supervisory, and communication skills
  • Ability to work independently

Experience

No previous experience in senior care required, but interest in or experience working with seniors would be an asset.

KEY ACTIVITIES

Direct Care and Monitoring of Residents

Administers oral, injectable, inhalation, transdermal, and topical medications as well as eye drops and eardrops, as prescribed, and accurately records in the MAR or eMAR.

Monitors and records vital signs, as required.

Completes resident health assessments — upon admission and when scheduled or required due to changes in resident's health.

Assesses reactions to medications.

Assesses residents' mental health, when required.

Assists with activities of daily living (ADLs) as per care plan (eg. nutrition, personal care, and mobility).

Development and updating of nursing care plans.

Provides bowel and bladder care (e.g. suppositories, enemas, catheters, ostomy bags)Transferring and repositioning of residents (e.g. to and from bed, chair, commode, bath chair) using proper body mechanics.

Monitors and ensures proper nutrition and hydration (e.g. assist / feed, cut meats, tube feeds). Observes, reports (written and / or shift), and records physical and psychological observations / changes in resident condition (e.g. skin, condition of feet and nails, mobility, nutrition, vital signs, and glucose levels).

Promotes the social, spiritual, and emotional well-being of residents; monitors and reports to the care team any changes in behaviour.

Promotes good communication with residents and their support systems. Assists / porters residents to activities, appointments, outings where required.

Responds to resident call systems.

Indirect Care

Accurately records information in resident charts in a timely fashion.

Files / maintains documentation (charts, notes, MAR, eMAR)

Supervises CCAs (Continuing Care Assistants) and Home Helpers.

Orders, maintains, and distributes inventory for resident usage (e.g. lotions, briefs, soaps, tissues) and medical supplies (e.g. exam gloves, dressing supplies, stock meds, injectable supplies, etc.).

Monitors food and fluid intake and output when required.

Records inventory items for billing purposes.

Collects specimens (e.g. urine, stool, and sputum) and readies for lab, when required.

Completes transfer / lift / repositioning and falls prevention assessments and posts appropriate logos.

Ill Related Key Work Activities

Disposes of contaminated items / waste and sharps as per protocol.

Communicates with physicians and implements physicians' orders, including accompanying visiting physicians to examine residents and assisting in physical exams.

Notifies Maintenance Department of required repairs to equipment.

Participates in Quality Improvement protocols, where required.

Communicates and participates in emergent situations as per protocol.

Communicates with outside agencies (Home Care, CPAS, etc.) and coordinates services.

Follows universal / standard precautions, infection prevention control, and isolation techniques.

Performs security checks, where required.

Other duties as assigned by CEO, ED, Director of Health and Wellness, Personal Care Home Manager.
