General Description

The Social Worker at the LTN Health Centre is a core member of a multi-disciplinary team dedicated to supporting the health and wellness of the Lheidli T’enneh community. This role is grounded in an Indigenous worldview, ensuring that services are culturally safe, respectful, and inclusive of Indigenous ways of knowing and being. The Social Worker provides holistic, client-centered care through advocacy, education, system navigation, and capacity building. They work collaboratively with clients, families, community partners, and leadership to develop and coordinate community-based services that respond to the evolving needs of the Nation.

Key Responsibilities:

Client Support & Case Management

  • Conduct comprehensive assessments (e.g., needs assessments, Persons with Disabilities [PWD] applications).
  • Develop and implement individualized care plans.
  • Maintain accurate records, case notes, and communication logs.
  • Provide direct support to clients through therapeutic and safety-focused interventions.
  • Assist clients with completing various forms, including government and service applications.

Advocacy & System Navigation

  • Advocate for clients’ rights and access to services.
  • Support clients in navigating complex social systems (e.g., housing, financial aid, disability services).
  • Guide clients through Band services and supports.
  • Liaise between clients, families, service providers, and community agencies to ensure coordinated care.

Education & Empowerment

  • Provide one-on-one and group education sessions to clients and community members.
  • Empower clients through guidance, information sharing, and skill-building.
  • Offer consultation and training to staff on Indigenous social issues and culturally safe practices.

Community Engagement & Program Development

  • Build trusting relationships with community members and service providers.
  • Develop and coordinate community-based programs in response to identified needs.
  • Collaborate with Nation leadership, Health Leads, and external partners to enhance service delivery.

Team Collaboration

  • Work effectively within a multi-disciplinary team to ensure integrated care.
  • Participate in team meetings, case reviews, and collaborative planning sessions.
  • Share knowledge and contribute to a supportive, culturally informed work environment.

Specialized Support Areas

  • End-of-Life Planning: Assist with Power of Attorney, representation agreements, and will preparation.
  • Youth-to-Adult Transitions: Support transitions to adult services such as CLBC and disability programs.
  • Transportation Support: Provide medical rides and assist clients in accessing essential services.

Skills and Qualifications

Education & Certification

  • Bachelor’s degree in social work.
  • Minimum 2 years of recent, related experience in a healthcare setting, working with medically complex clients and those with concurrent disorders.
  • Equivalent combination of education, training, and experience may be considered.
  • Advanced training in threat/risk assessment and/or trauma response.
  • Current registration with the BC College of Social Workers as a Registered Social Worker (RSW).
  • Valid Class 5 B.C. Driver’s License and willingness to provide a driver’s abstract.

Professional Experience

  • Proficient in various meeting and coordination modalities.
  • Skilled in using assessment and screening tools for evaluating suicide and violence risk.
  • Knowledgeable about socio-historical events relevant to First Nations in Canada.
  • Familiarity with traditional healing practices, customs, values, and traditions of local First Nations.
  • Experience in applying critical reflection within professional practice.

Relationship Building: Proven ability to establish strong working relationships with allied professionals, community leaders, knowledge keepers, and Elders.

Teamwork: Demonstrated ability to foster teamwork and uphold excellence in client care.

Technical Proficiency: Skilled in using Meditech, Windows-based programs, email, word processing, and data analysis tools.

Physical Capability: Able to perform the physical duties required by the position.
