What would my role be?
Reporting to the Medical Director/Clinic Supervisor, you will act as the community liaison for the family medicine clinic. This includes providing health care and social services navigation, supporting patients with forms completion (ie: ODSP self-report, disabled parking permit, Para-transpo), resource identification and how to access community resources, organizing case conferences, support to patients with referrals for physiotherapy, dental and optometry, and follow-up support for those having difficulties attending appointments.
How do I qualify?
Required:
- Completion of High School Diploma.
- Minimum of one (1) year of relevant experience in typing, computer and switchboard.
- Fluent in French and English (Candidates who do not meet this language requirement are eligible to apply for this position and may be considered if there are no other suitable applicants who meet this requirement).
Preferred:
- Two (2) year college diploma in a related field.
- Fluency in another language.
What skills should I possess to succeed in the role?
- Typing and computer work.
- Telephone answering and appropriate redirection of calls, mail, faxes.
- Excellent personal communication skills.
- Be able to work in an inter-professional team environment.
- Be able to prioritize and be flexible in a very active work environment.
- Accuracy and attention to detail.
- Intermediate computer skills.
- Advanced knowledge of medical terminology.
