The Opportunity
We are looking for a Team Based- Care Manager to join our Team at CUPS.
- Location: Onsite, CUPS - 1001 10 Ave SW, Calgary
- Department: CUPS Primary Care Services
- Reports to: Director of Primary Care Services
- Schedule: Full-time, Mon to Fri, 37.5 hours per week
- TBC Reports: Team Lead of TBC Medical Office Assistants, Team Coordinators, and the Team Lead of Nursing and Allied Health On-site
Position Summary
The Team Based‑ Care (TBC) Manager is an operational leader responsible for advancing interdisciplinary, patient-centered primary care across CUPS Primary Care Services, including the Access Care Clinic (ACC), Prenatal & Family Health Clinic (PFHC), and Primary Care Clinic. This role plays a central role in coordinating, implementing, and optimizing the team-based care model in a manner that improves clinical outcomes and enhanced client experience. The TBC manager will be responsible for providing leadership and guidance to the on-site nursing and allied health teams, as well as providing leadership and guidance to support the delivery of clinical services in the CUPS Health Clinic. This role ensures that clinicians, nurses, allied health professionals, and administrative staff work collaboratively to deliver comprehensive, proactive, and accessible care within the Access Care Clinic (ACC), Prenatal & Family Clinic and Primary Care Clinic.
The TBC Manager supports panel management, integration of social services, optimization of chronic disease management, preventative screening, care transitions, quality improvement, and the integration of new care team roles. As the clinical leader for the on-site nursing and allied health teams, the TBC Manager is responsible for fostering integrated, team-based care, supporting professional practice, and advancing evidence-informed approaches to care delivery. This position helps CUPS Primary Care Services evolve toward advanced team-based primary care models, improving outcomes and reducing system inefficiencies for both clients and staff.
Key Responsibilities
- Lead the design and optimization of workflows supporting interdisciplinary care (RN, LPN, NP, behavioural health, social work, pharmacy, dietitians, etc.).
· Provide overall management and direction to the on-site CUPS nursing team and allied health team through direct management of the Team Lead of Nursing and Allied Health Onsite
- Collaborate with Manager of Clinic Operations to ensure seamless function, support, and oversight of clinic administration roles.
- Implement the team-based care workflows for PC teams and ACC
- Provide coaching, development and communication
- Develop and refine care pathways, and standardized processes including integration across CUPS programs.
- Develop and maintain a compassionate, professional, and effective healthcare team via effective recruitment, training, and engagement strategies
- Support integration of new team roles and evolving scopes of practice.
- Facilitate daily/weekly team huddles or client care planning discussions.
- Work closely with clinicians, Clinic Operations Manager, TBC Coordinators, Practice Facilitator, and allied health providers to identify gaps in care, unmanaged conditions, or follow-up needs.
- Implement and support the use EMR tools such as registries, reminders, and analytics dashboards to track care needs and outcomes.
- Ensure TBC Coordinators engage patients in shared decision-making and self-management strategies.
- Assist clients to access additional, relevant CUPS programing, resources, social supports, and preventative care.
- Lead and participate in QI working groups in primary care
- Contribute to clinic performance improvement initiatives aligned with PCN, provincial, and organizational priorities.
- Collect, interpret, and report data related to access, and care gaps.
- Support implementation of evidence-based guidelines and provincial primary care models.
- Ensure teams have accurate, timely documentation in the EMR.
- Support reports for panel stratification, recalls, screening programs, and care optimization.
- Support onboarding and training of administration and Medical Office Assistant staff on EMR workflows and team-based functionality.
- Contribute to safety and wellness of clients, colleagues and visitors by remaining up to date on supports teams to adhere to CUPS health & safety protocols
- Support program reporting requirements, ensuring teams are maintaining data integrity & availability for evaluation teams
- Build collaborative relationships, internally and externally, with community health, mental health, home care, social services, and specialty clinics.
- Enhance continuity of care across the patient’s entire health system journey.
- Support operational and budget planning for team-based care initiatives
Employment Requirements
Qualifications
- Bachelor’s degree in nursing and registration with CRNA is required
- Master’s degree in health administration, public health, or related field is an asset.
What You Bring
- 6-8+ years experience in primary care, community health, or care coordination.
- 1-2 years experience in working in a leadership capacity, with demonstrated skill in coaching and development
- Experience working in a Patient Medical Home or team-based care environment is preferred.
- Knowledge of chronic disease management and population health principles.
Skills & Competencies
- Strong understanding of primary care workflows, scopes of practice, and interdisciplinary teamwork.
- Commitment to patient-centred, culturally safe, and trauma-informed care.
- Excellent communication, facilitation, and relationship-building skills.
- Ability to manage multiple priorities in a fast-paced clinic setting.
- Proficiency in EMRs, population health tools, and data-informed care planning.
Working Conditions
- Works in a collaborative primary care clinic environment with frequent interaction across disciplines.
