About the Peterborough Family Health Team
Peterborough Family Health Team (PFHT) is a collaborative primary care organization that works alongside family physicians and community partners to provide comprehensive, team-based care to the residents of Peterborough and Peterborough County. Our interdisciplinary team works together across multiple clinical sites.
PFHT supports clinical excellence, innovation, and digital health solutions that enhance patient care, improve provider workflows, and protect the privacy and security of health information.
We are currently seeking multiple contract positions for 30-37.5 hours/week (0.8 – 1.0 FTE full-time) to support the expanding needs of the organization and to ensure consistency in service delivery. These contracts are expected to go to March 31, 2027.
Overview
The Registered Practical Nurse (RPN) reports to a Clinical Manager at the Peterborough Family Health Team (PFHT) and is an integral part of an interprofessional, multidisciplinary team who works co-operatively with all members of the healthcare team in the promotion of patient health and prevention of disease. Practicing within their full scope as defined by the College of Nurses of Ontario (CNO) Standards of Practice, the RPN supports the delivery of high-quality, person centered cared across a variety of clinical and preventative services.
Responsibilities
The Registered Practical Nurse (RPN) will support the Family Health Organization (FHO) through population health management, preventive care, and clinical operational support, practicing within the College of Nurses of Ontario (CNO) Standards of Practice. Working collaboratively with physicians and the interdisciplinary team, the RPN will help improve patient outcomes, address care gaps, and support continuity of care. Key responsibilities include:
- Conduct population health reviews using the electronic medical record (EMR) to identify patients requiring preventive care, chronic disease monitoring, and follow-up.
- Coordinate preventive care initiatives, including cancer screening, abdominal aortic aneurysm (AAA) screening, bone mineral density (BMD) screening, immunizations, and other evidence-based health maintenance activities.
- Reconcile referrals, consultations, diagnostic testing, and outstanding follow-up to support continuity and timely access to care.
- Contact patients to coordinate appointments, screening, investigations, and follow-up care.
- Support physicians by reviewing charts, preparing clinical information, and identifying gaps in care.
- Provide clinical nursing support across participating FHO offices as required, including coverage for planned and unplanned staff absences and periods of increased operational need.
- Perform clinical nursing duties within the RPN scope of practice, including immunizations, injections, wound care, vital signs, specimen collection, patient education, and other delegated nursing activities.
- Collaborate with physicians, nurse practitioners, medical office assistants, and the interdisciplinary team to coordinate patient care and support clinic operations.
- Maintain accurate and timely documentation in the EMR in accordance with professional, organizational, and legislative requirements.
- Participate in quality improvement, data collection, infection prevention and control, and other duties as assigned to support the effective operation of the FHO and PFHT.
Position Requirements
- Ability to perform all clinical and technical tasks within the RPN scope of practice
- Strong interpersonal and communication skills with sound clinical judgment
- Ability to work independently and collaboratively within a busy, multidisciplinary environment
- Proficient in the use of Primary Care Electronic Medical Records (e.g. Myle, Telus PS Suite, Accuro) and other relevant digital tools (e.g. Ocean, Connecting Ontario)
- Understanding of how social determinants of health impact individual and community health
- Demonstrated commitment to anti-racism, anti-oppression, equity, and inclusion principles
- Demonstrated problem-solving skills and adaptability in a fast-paced environment
- Familiarity with primary care and how it intersects with other parts of the healthcare system
- Willingness to work in full scope of practice, performing delegated acts through medical directives
- A valid driver’s license and reliable vehicle (travel to various clinic locations may be required)
- Clear vulnerable sector police check
- Willingness to participate in data collection for quality improvement and program development
- Ability to lift up to 20 lbs
- Flexibility to adapt to changing hours and clinical demands
Hours of Work:
Hours of service are pre-established per employment agreement and may require some evenings and weekends. Any deviation from the established schedule shall not negatively impact upon the provision of service to the patient population.
Qualifications
- Diploma in Practical Nursing
- Minimum 3 years’ clinical experience, preferred primary care
- Registered in good standing with the College of Nurses of Ontario (CNO)
- Active knowledge of CNO Standards and Guidelines
- Excellent documentation and time management skills
- Experience in immunization administration
- Well-women’s and well-baby assessment skills an asset
- Member of WeRPN preferred
