Full job description

Description:

This position supports multiple PCN Medical Homes on a rotating basis. The Panel Coordinator (PC) works collaboratively with physicians, medical home staff, and interdisciplinary team members, with a combination of in-clinic, central site, and remote work. Using the medical home’s Electronic Medical Record (EMR), the PC identifies and contacts patients due for preventive screenings, follow-up care, or appointments. Responsibilities may also include chart reviews, report generation, and patient outreach to support proactive care and panel management.

Salary Range: $23.20- $30.28 / hour

Closes: 24 July 2025

QUALIFICATIONS:

Education and Certifications

  • Medical Office Assistant/Unit Clerk certification preferred
  • Medical terminology course
  • Combination of equivalent education and experience may be considered
  • Valid driver’s license and access to vehicle

Experience

  • Previous experience in primary care setting preferred
  • Knowledge of Electronic Medical Record system and NetCare an asset

Skills and Attributes

  • Excellent oral and written communication skills
  • Ability to balance multiple priorities and use problem-solving skills
  • Ability to work independently
  • Excellent computer knowledge, including M365 applications
  • To support the vision and mission of the Primary Care Network
