Overview
Join our dynamic healthcare team as a Primary Health Care Nurse Practitioner, where your expertise will directly impact patient well-being across diverse populations. In this vital role, you will provide comprehensive primary care services, including diagnosis, treatment, and health education, in a collaborative environment that values innovation and compassionate care. Your proactive approach will help promote health, prevent illness, and manage chronic conditions effectively, ensuring patients receive the highest standard of personalized care.
Responsibilities
- Conduct thorough patient assessments, including history taking and physical examinations for individuals across all age groups, with special attention to pediatrics and senior care.
- Develop and implement individualized treatment plans, including medication management and pain control strategies, utilizing your expertise in medical terminology and clinical protocols.
- Administer medications safely and accurately, including injections, phlebotomy procedures, and laboratory specimen collection, adhering to best practices in medication administration.
- Manage acute care cases efficiently by diagnosing illnesses, ordering diagnostic tests such as laboratory work or imaging studies via electronic health record systems like Epic or Cerner.
- Coordinate case management efforts for patients with complex needs such as dementia care or assisted living requirements, ensuring seamless continuity of care across settings like nursing homes or home care environments.
- Educate patients and their families on health promotion, disease prevention, medication use, and lifestyle modifications to foster long-term wellness.
- Document all patient interactions meticulously within medical office systems while maintaining compliance with healthcare regulations and confidentiality standards.
Skills
- Proven experience working with children (pediatrics) and senior populations in diverse settings such as hospitals, nursing homes, or assisted living facilities.
- Strong background in acute care environments with the ability to handle urgent cases confidently.
- Expertise in pain management techniques and chronic disease management strategies.
- Skilled in case management practices to coordinate comprehensive patient care plans effectively.
- Familiarity with electronic health record platforms such as Epic or Cerner for documentation and order entry.
- Excellent knowledge of medical terminology combined with proficiency in medication administration procedures.
- Hands-on laboratory experience including phlebotomy and specimen handling; laboratory testing knowledge is a plus.
- Experience providing dementia care or home healthcare services is highly desirable.
- Ability to communicate clearly with multidisciplinary teams while demonstrating compassion for patients’ needs.
