Are you a mature, skilled nurse looking for a fulfilling change of pace?

We're seeking dedicated nurses who want to provide high-quality, one-on-one care to a single client. This role provides the stability and consistency often missing from hospital work, with a focus on building a strong rapport with your patient and their family.

Schedule will be:

  • Day & evening shifts, including every 2nd weekend.

What You’ll Be Doing:

  • Deliver comprehensive nursing care to patients in home settings, focusing on individualized treatment plans.
  • Conduct thorough assessments of patients' health status and develop appropriate nursing interventions.
  • Collaborate with multidisciplinary teams to coordinate patient care and ensure continuity of services.
  • Educate patients and their families on health management, medication administration, and home care practices.
  • Monitor patient progress and adjust care plans as necessary based on clinical evaluations.
  • Maintain accurate documentation of patient care activities using electronic health record systems, such as Epic.
  • Provide support for laboratory tests and procedures as needed, ensuring adherence to safety protocols.

What we look for in our candidates:

  • Experience with central venous catheters and Total Parenteral Nutrition (TPN).
  • Competency in G-tube care and gastric aspiration.
  • Experience providing ostomy care and recording/monitoring output.
  • Experience assisting with ADLs while promoting client independence: could include personal care such as toileting and dressing.
  • Strong communication skills and ability to manage age-appropriate behaviours.
  • Familiarity with home medical equipment (CADD pump).
  • Knowledge of emergency procedures.
  • Minimum of 2 years of Home and Community Care experience
  • Minimum of 2 years Pediatric Experience.

Pay: $32.00-$35.00 per hour
