Full job description

We are seeking an Intake Coordinator to serve as the first point of contact for children, youth, and families seeking rehabilitation and developmental services. The Intake Coordinator will be responsible for coordinating the intake and referral process, responding to inquiries, guiding families to appropriate programs and services, and supporting timely access to care. Working collaboratively with an interdisciplinary team, this role will ensure a seamless, child and family-centred experience while contributing to the delivery of high-quality paediatric rehabilitation services.

Boomerang Health, powered by SickKids, is a multidisciplinary paediatric clinic, independent but wholly owned by The Hospital for Sick Children (SickKids). We offer physician, rehabilitation, and developmental services up to the age of 18.

Job Summary

  • First point of contact for rehabilitation and developmental services inquiries; in person, email and telephone;
  • Communicate with the general public, families, and referring sources to respond to inquiries;
  • Required to independently assess and evaluate need for services and make recommendations for additional services as appropriate;
  • Manage, organize and track all incoming referrals and intake communications;
  • Receive and assess the appropriateness of requests for service using criteria provided by Boomerang Health team leads, and in consultation with members of the clinical team;
  • Create and maintain confidential patient records in the Electronic Medical Record (EMR) system, containing all consent forms, test results and relevant documentation;
  • Secure and coordinate necessary supporting demographic and health documentation, following all privacy rules and regulations;
  • Manage intake inquiries, appointment scheduling, and service waitlists, ensuring families are kept informed of anticipated wait times and that all referrals and inquiries are tracked through to completion;
  • Make recommendations for improving processes and systems;
  • Data entry and management of the referral tracking systems;
  • Undertake small projects/assignments as requested;
  • Redirect less appropriate cases to other agencies, hospitals or clinics and assist in finding alternative resources in the community;
  • Prepare monthly statistical and annual reports;
  • Provide front desk reception coverage as required; and
  • Other duties as assigned.

Qualifications

Postsecondary degree or diploma in Psychology, Sociology, Child and Youth Care, Health Sciences, Rehabilitation, Health Administration, or a related field;

Minimum of 3–5 years of experience in intake, care coordination, client navigation, or healthcare administration, preferably in paediatric rehabilitation, children’s treatment centres, developmental services, or community healthcare;

Accounting knowledge and experience is a strong asset; and

Experience in using an electronic medical record system.

Experience/Skills

  • Highly developed administrative and organizational skills with the ability to handle multiple priorities, work under pressure and meet strict deadlines;
  • A self-starter willing to take initiative, with proven problem-solving and critical thinking abilities, managing effectively in a dynamic, changing environment;
  • Excellent time management skills and attention to detail;
  • Ability to comprehend and act on complex and sensitive matters with professional tact, diplomacy, and confidentiality;
  • Exceptional interpersonal skills with children and families;
  • Superb ability to collaborate with colleagues and peers;
  • Strong communication and grammar skills, both verbal and written;
  • Proficiency in the use of computer hardware and software, particularly in Microsoft Word, Excel, PowerPoint and Outlook; and
  • Knowledge of medical terminology is an asset
