Reporting to the Manager, Access & Flow at SMH, the Patient Flow Specialist will be responsible for expediting safe patient placement through constant coordination, communication, and prioritization of all patients at SMH. A strong focus will be predicting and overcoming barriers to patient flow.

Accountabilities:

  • Accountable for patient placement throughout the Hospital and has the authority to determine bed assignment and provide assistance to individuals assigning beds.
  • Facilitates daily bed meetings, advises stakeholders of overall corporate bed status, and works with interprofessional teams to follow up on issues or barriers to patient flow and discharge, throughout the day
  • Collaborates and liaises between all patient care services, medical staff, bed management teams, and hospital support programs and services, to ensure that patients are placed in the most appropriate available bed
  • Assists with coordination of internal transfers of patients between units and transfers of external patients between facilities.
  • Develops and nurtures partnership with internal and external colleagues.
  • Contributes to a culture of inquiry and innovation.
  • Collects data related to quality, utilization, bed readiness performance, and other outcome indicators
  • Assists with developing and implementing innovative plans to achieve required Access, Flow and Transitions targets
  • Strives to strengthen and simplify systems/structures/policies to support the vision of providing the best care experiences for patients and families
  • Identify opportunities for patient flow improvements and act as a subject matter expert for patient flow projects and initiatives.
  • Role models mature and professional behavior at all times; supports the development of teamwork across the Hospital

Qualifications:

  • Regulated Health Professional required
  • Master's degree an asset
  • Minimum 3-5 years recent acute care experience
  • Previous leadership experience a strong asset
  • Excellent conflict resolution skills
  • Proactive nature with a drive to investigate and resolve problems.
  • Demonstrated evidence of innovative leadership and critical thinking, with proven ability to foster a collaborative team approach to decision making
  • Demonstrated ability in communication, team building, problem solving and change management within an interprofessional environment
  • Strong understanding of bed management/utilization and patient flow methodologies as they relate throughout the continuum of care
  • Understanding of continuous quality improvement tools and methodologies
  • Intermediate level computer skills (MS Office, email, electronic health record system.) Experience with bed management software a strong asset
  • Evidence of continuing professional/personal development
  • Demonstrated excellent communication skills
  • Excellent attendance record
