PHYSICIAN ASSISTANT (PA) — FAMILY PRACTICE

Ajax, Ontario | Full-Time

Join a collaborative, well-supported primary care team where you'll play a key role supporting a family physician and delivering high-quality, community-based care.

Ajax Family Practice & Walk-In Clinic is seeking a Physician Assistant (PA) to work closely with one of our family physicians in a structured, physician-led primary care model.

The Role

This position focuses on supporting a dedicated family physician by providing care to their rostered patients under physician supervision. You'll play an important role in follow-ups, chronic disease management, preventive care, and same-day access while helping maintain continuity of care for an established patient population.

Responsibilities

  • Conduct comprehensive patient assessments, obtain medical histories, perform physical examinations, and develop management plans in collaboration with the supervising physician
  • Support the ongoing care of patients from an established physician's roster, helping maintain continuity and strong long-term patient relationships
  • Manage routine primary care visits, follow-ups, preventive care, and chronic disease management for patients across the lifespan
  • Support same-day and urgent patient needs, including episodic care and physician coverage as delegated
  • Order and interpret diagnostic tests and investigations in accordance with Ontario legislation, medical directives, and supervising physician delegation
  • Develop and implement treatment plans, including prescribing medications where authorized under medical directives or delegation
  • Provide patient counseling and education on treatment plans, medications, preventive care, lifestyle modifications, and community resources
  • Perform office-based procedures within scope, training, and physician delegation
  • Deliver compassionate, patient-centered, and culturally responsive care tailored to each individual's needs and circumstances
  • Collaborate closely with the supervising physician and clinic team to coordinate care, ensure appropriate follow-up, and support high-quality patient outcomes
  • Maintain accurate and timely clinical documentation within the OSCAR EMR system
  • Contribute to a positive, supportive, and team-oriented clinic environment focused on excellent patient care
  • Assist with preventive care initiatives, patient outreach, and chronic disease monitoring to help improve overall patient health outcomes

Required

  • Graduate of an accredited Physician Assistant education program recognized in Canada
  • Eligible for or currently certified through the Physician Assistant Certification Council of Canada (PACCC) and/or the National Commission on Certification of Physician Assistants (NCCPA)
  • Eligible to practice as a Physician Assistant in Ontario in accordance with current provincial legislation and clinic requirements
  • Current Basic Life Support (BLS) certification
  • Minimum of one (1) year of recent primary care or family medicine experience preferred
  • Excellent patient assessment, clinical reasoning, and communication skills
  • Experience with OSCAR EMR is an asset
