Title: Clinical Coordinator – Family Medicine Clinic

The Weeneebayko Area Health Authority (WAHA) is seeking an experienced professional to fill the key role of Clinical Coordinator in the Family Medicine Clinic.

What You’ll Do

  • Lead patient care in the Family Medicine Unit
  • Ensures referrals, appointments and consultations are arranged and followed up on
  • Maintain critical communication between department staff and patients
  • Work with department manager on program development, patient outcome improvement, as well as a strong customer service experience
  • Effectively manage resources, including people, equipment and facilities
  • Coordinates a multidisciplinary team to provide high quality patient care

What You Bring

  • Designation as a Registered Nurse with the College of Nurses of Ontario in good standing
  • Diploma in Nursing is required, a Bachelor of Science in Nursing is preferred
  • Experience as a Nurse in a Family Medicine/Primary Health Team setting
  • Ability to act as a team lead for the staff in the department
  • Experience working in the James Bay Coast is an asset
