Licensed Practical Nurse (LPN) – Patient Care Coordinator | Phlebotomy & IV Therapy

About the Role

Primary MD and Sublime Life are a physician-led clinical practice and wellness space focused on personalized medicine, preventive health, longevity medicine and an elevated patient experience.

We are looking for an experienced Licensed Practical Nurse (LPN) who enjoys both hands-on clinical care and being an integral part of the patient experience.

This is a unique hybrid role combining nursing, phlebotomy, IV therapy and clinical testing with patient coordination and front-of-clinic responsibilities.

We are looking for someone who is equally comfortable drawing blood, starting an IV and supporting clinical protocols as they are greeting patients, answering the phone, coordinating appointments and making sure patient care is followed through from beginning to end.

This is not a traditional bedside nursing position, nor is it solely an administrative role. It is ideal for an LPN who enjoys variety, autonomy, organization and developing meaningful relationships with patients in a boutique physician-led practice.

The position is primarily based within Primary MD, supporting medical care delivery and patient coordination, with exposure to the broader Sublime Life longevity and wellness environment.

Key Responsibilities

  • Perform phlebotomy, including difficult draws; pediatric draw experience is an asset
  • Initiate and monitor IV therapy within LPN scope, training and clinic protocols
  • Perform and assist with clinical testing including ECGs and other in-office diagnostics
  • Coordinate specialized laboratory and longevity testing, including specimen collection, preparation, documentation and shipping
  • Maintain accurate clinical documentation within the EMR
  • Track investigations and results to ensure appropriate review and closed-loop follow-up
  • Support physicians with day-to-day clinical care and procedures
  • Assist with medication, injection and treatment protocols within appropriate LPN scope and physician direction
  • Maintain inventory of medications, IV supplies, laboratory kits and clinical supplies
  • Serve as a key point of contact for patients both in person and by telephone
  • Welcome and check in patients and assist with patient intake
  • Coordinate appointments, testing, referrals and follow-up care
  • Respond to patient requests and ensure outstanding items are appropriately actioned
  • Help maintain smooth patient flow throughout the clinic
  • Participate in front-of-clinic responsibilities, including telephone coverage, scheduling and patient coordination
  • Work collaboratively with physicians and the broader Primary MD | Sublime Life team
  • Help identify opportunities to improve clinical workflow, organization and the patient experience

Qualifications

Required:

  • Current registration and good standing as a Licensed Practical Nurse (LPN) in Alberta
  • Demonstrated phlebotomy competency and confidence performing blood draws
  • Training and/or experience with IV initiation and monitoring
  • Strong clinical judgement and understanding of LPN scope of practice
  • Comfortable working within an EMR and maintaining accurate clinical documentation
  • Excellent organizational skills and attention to detail
  • Ability to independently manage multiple priorities and maintain follow-through
  • Excellent verbal and written communication skills
  • High degree of professionalism, discretion and respect for patient confidentiality
  • Comfortable participating in both clinical and front-of-clinic responsibilities
  • Genuine commitment to exceptional patient care

Assets:

  • Experience in a family medicine, primary care, executive health, outpatient or specialty clinic
  • Experience with difficult and/or pediatric blood draws
  • Experience with IV therapy
  • Experience coordinating specialized laboratory testing
  • Familiarity with preventive health, longevity medicine or integrative health
  • Previous experience combining nursing and patient-care coordination responsibilities
