Clinical Nurse Coordinator & Operations Lead

Well Approach Centre | Burlington, Ontario

Job Overview

Well Approach is seeking a highly organized, proactive and technology-savvy Registered Practical Nurse (RPN) to join our team as Clinical Nurse Coordinator & Operations Lead.

This is a unique role combining clinical nursing, patient care coordination, clinic operations, technology, longevity medicine and medical aesthetics.

The successful candidate will play a central role in ensuring patients receive a seamless, highly personalized experience from initial assessment through testing, treatment and follow-up. They will also help manage the day-to-day clinical operations of the practice and develop efficient systems as Well Approach grows.

We are looking for someone who is independent, resourceful and exceptionally strong at follow-through. The ideal candidate sees what needs to be done, takes ownership and completes the task without requiring constant direction.

An interest in longevity, functional medicine, preventative health, medical aesthetics and emerging healthcare technology is strongly preferred.

Key Duties & ResponsibilitiesClinical & Patient Care

  • Conduct patient intake and nursing assessments
  • Review medical history, medications, supplements and allergies
  • Obtain vital signs and perform appropriate clinical assessments
  • Prepare patients and relevant information for physician consultations
  • Maintain accurate and timely clinical documentation
  • Identify clinical concerns and escalate appropriately to the physician
  • Provide patient education and treatment instructions
  • Assist with physician-led procedures and treatments
  • Perform nursing procedures within scope of practice and training
  • Assist with IV therapy, injections, phlebotomy and patient monitoring where appropriate
  • Maintain infection-control and clinical safety standards

Patient Care Coordination

  • Act as a central point of contact throughout the patient's care journey
  • Coordinate laboratory testing, imaging, referrals and specialized investigations
  • Track outstanding test results and follow-up requirements
  • Coordinate appointments and multidisciplinary care
  • Ensure physician recommendations and treatment plans are implemented
  • Monitor patient progress and scheduled reassessments
  • Follow up with patients after appropriate treatments and procedures
  • Identify missed appointments, incomplete testing or gaps in care
  • Ensure patients understand their treatment plan and next steps
  • Prevent patients from becoming lost to follow-up

Longevity & Health Assessment

Assist with and learn advanced health, longevity and performance testing, which may include:

  • InBody body-composition analysis
  • Grip-strength testing
  • Functional strength and mobility assessments
  • Balance testing
  • Sit-to-stand testing
  • VO₂ and cardiovascular fitness assessment
  • Lactate testing
  • HRV assessment
  • ECG screening
  • Blood-pressure and vascular assessments
  • Cognitive assessments
  • Biological-aging testing
  • Advanced laboratory and functional medicine testing

Training will be provided where appropriate.

Medical Aesthetics

  • Assist with aesthetic consultations and patient preparation
  • Complete treatment intake and screening
  • Take standardized clinical photographs
  • Review treatment contraindications
  • Prepare treatment rooms and equipment
  • Assist with procedures
  • Provide pre- and post-treatment education
  • Coordinate treatment plans and follow-ups
  • Learn the medical aesthetic technologies offered by Well Approach

Technology & Digital Systems

The successful candidate will be expected to become highly proficient with the clinic's technology and digital workflows.

Responsibilities include:

  • Using Practice Better EMR
  • Managing digital intake forms and patient information
  • Using online scheduling and patient communication systems
  • Managing electronic consent forms
  • Supporting telemedicine workflows
  • Using Google Workspace and digital task-management tools
  • Learning AI-assisted clinical and administrative tools
  • Helping implement automated patient communication
  • Identifying opportunities to use technology to improve efficiency and patient care

Clinic Operations

  • Coordinate the daily clinical workflow
  • Review the patient schedule and ensure the team is prepared
  • Ensure treatment rooms and equipment are ready
  • Monitor outstanding clinical and administrative tasks
  • Maintain clinical supplies and inventory
  • Monitor medications and supply expiry dates
  • Coordinate ordering of clinical supplies
  • Manage laboratory and testing kits
  • Maintain clinic checklists, protocols and procedures
  • Help develop and implement Standard Operating Procedures (SOPs)
  • Identify operational inefficiencies and recommend solutions
  • Improve communication and workflow between team members
  • Help standardize patient pathways and assessment protocols
  • Support implementation of new services, technologies and programs

Physician Support

A key responsibility of this position is helping protect physician time and improve clinical efficiency.

The Clinical Nurse Coordinator will:

  • Prepare relevant information before physician consultations
  • Organize patient questions and clinical information
  • Ensure physician recommendations are implemented
  • Handle appropriate nursing matters independently
  • Escalate matters requiring physician involvement
  • Provide concise updates on patient issues
  • Follow through on physician-directed tasks
  • Reduce unnecessary interruptions and administrative workload

RequirementsRequired

  • Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario
  • Current CPR/BLS certification
  • Strong nursing assessment skills
  • Excellent organizational and time-management skills
