Case Manager RN Temporary 18 Month Contract

Position Summary:

As a key member of the healthcare team under the direction of Executive and Office Manager. The Case Manager RN works closely with patients, families and caregivers to organize the patient’s journey through there surgical journey at the clinic.

Responsibilities:

Administration

Develop individualized plan of care with patients and the family,

Work directly with surgeon to plan the resource needs of the client

  • Provide patient and family teaching
  • Communicate effectively with patient, family, team members and external health care providers involved in the planning and care of the patient
  • Provide positive leadership to team, patient, family and other healthcare providers along the continuum
  • Participate in staff development, team meetings, staff meetings and education of colleagues.
  • Coordinate resources and appointments with patient and external stakeholders
  • Prepare charts and ensure they are complete
  • Patient advocate
  • Ensures patient is prepared medically for surgery (including Internal Medicine and
  • Anesthesia appointments, blood work and x-rays etc. as necessary).
  • Coordinate wait-list management
  • Assist with collection of data for Key Performance Indicators as requested by Management Team
  • Prioritize and respond to phone calls and messages in a timely, required 24-hour response time

Education

Nursing Degree

Experience

  • Minimum 3 year in a medical administration using electronic medical records software
  • Solid understanding of the Health Information Act, patient privacy law and FIOP
  • Intermediate level knowledge of medical terminology
  • Solid skills in using software such as MicroSoft Office
  • Excellent understanding of Physician & RN instructions to patients in written format
  • Experience using Accuro EMR is an Asset
