Case Manager

Home Instead is an Independent Home Care Provider, delivering high-quality care to our Senior Clients who desire to remain at home, instead of moving to a care community. We are proud of our reputation of excellence in the community.

Home Instead Calgary is seeking a competitive and highly organized Case Manager to support seniors in maintaining their independence at home. This role serves as the primary point of contact for clients, families, caregivers, and community partners, ensuring the delivery of exceptional, client-centered care.

Key Responsibilities

  • Serve as the first point of contact for prospective clients and their families, providing guidance and support throughout the care journey.
  • Conduct client consultations and in-home assessments.
  • Develop, implement, and review individualized care plans.
  • Monitor client well-being and respond proactively to changing care needs.
  • Maintain regular communication with clients, families, and caregivers.
  • Complete quality assurance visits and client satisfaction reviews.
  • Maintain accurate and timely client documentation.
  • Collaborate with healthcare professionals, community organizations, and referral partners.
  • Provide caregiver support and coaching as required.
  • Participate in an on-call rotation.1 evening per week, 1 weekend in 5

Qualifications

  • Diploma or degree in Nursing, Social Work, Gerontology, Health Care Management, Human Services, or related field.
  • Minimum of two years of experience in case management, healthcare, home care, senior care, hospitality management, or a related field.
  • Strong understanding of aging, dementia care, chronic conditions, and community resources.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Strong organizational skills and attention to detail.
  • Proficiency with Microsoft Office and client management systems.
  • Valid Alberta driver's licence and reliable transportation.

Preferred Qualifications

  • Experience in home care, senior care, hospitality, or community-based healthcare services.
  • Knowledge of Alberta healthcare systems and community support programs.
  • Experience conducting client assessments and developing care plans.
