Virtual Registered Nurse, Case Manager - Women's Health & Lifestyle Medicine

About the Opportunity

We are seeking compassionate and experienced Registered Nurses to join a growing virtual healthcare team supporting women through various stages of their health journey.

This role combines virtual nursing, case management, patient education, and lifestyle-focused care. The successful candidate will support patients through ongoing care plans, health coaching, care coordination, and evidence-based education, helping individuals achieve meaningful and sustainable health outcomes.

Key Responsibilities

  • Serve as a primary point of contact for patients throughout their care journey.
  • Provide virtual case management and care coordination through phone and digital platforms.
  • Conduct nursing assessments and identify patient needs, goals, and barriers to care.
  • Develop and support individualized care plans in collaboration with healthcare providers.
  • Deliver education and coaching related to women's health, wellness, preventative care, and healthy lifestyle practices.
  • Support patient engagement, adherence to treatment plans, and achievement of health goals.
  • Coordinate referrals, follow-ups, and communication among interdisciplinary care teams.
  • Monitor patient progress and escalate clinical concerns as appropriate.
  • Maintain accurate and timely documentation within electronic medical record systems.
  • Advocate for patients and support a positive, patient-centered care experience.

QualificationsRequired

  • Current registration as a Registered Nurse (RN) in Canada.
  • Active licensure in at least two of the following provinces: Ontario, Quebec, Alberta British Columbia
  • Minimum of 2 years of recent nursing experience.
  • Strong clinical assessment, communication, and patient education skills.
  • Comfortable working independently in a fully virtual environment.
  • Experience utilizing EMRs and other healthcare technology platforms.
  • EPIC experience strongly preferred.

Preferred Assets

  • Experience in case management, care coordination, patient navigation, or chronic disease management.
  • Previous virtual care, telehealth, or remote patient support experience.
  • Experience supporting patients in women's health settings, including:
  • Perimenopause and menopause
  • Reproductive and sexual health
  • Preventative health and screening
  • Hormonal health
  • Weight management and metabolic health
  • Training or experience in lifestyle medicine, health coaching, motivational interviewing, or behaviour change methodologies.
  • Bilingual French and English language skills are considered a strong asset.
