Who We Are:
LOFT Community Services is a unique and dynamic charitable organization that supports people living with complicated issues such as mental and physical health challenges, substance use challenges, poverty and homelessness. Serving approximately 19,534 annually and providing 1,839 units of supportive housing last year, LOFT is one of Ontario’s largest mental health service providers of its kind. LOFT is also recognized as a Nonprofit Employer of Choice™ Award (NEOC) recipient.
The Path Home - Champagne program at Bathurst is a community-based inpatient program operating in partnership with the Centre of Addictions and Mental Health (CAMH). It provides 36 ALC (Alternate Level of Care) transitional beds for medically stable clients preparing for reintegration into the community. Clients benefit from case management, care coordination, and individualized transition planning, supported by 24/7 nursing and Personal Support Worker (PSW) services. On-site services include primary care, psychiatry, and medication support, while additional services such as foot care and occupational or physiotherapy are arranged through community referrals as needed. The program supports clients in building the skills and accessing the services needed to transition successfully to permanent housing or their designated discharge destination.
Job Summary:
The Life Enrichment Worker (LEW) is responsible for working within an interdisciplinary team and enhancing the quality of life for older adults and seniors living within a LOFT Program who are facing complex challenges. These complex challenges could include mental health and/or addiction challenges, cognitive impairment, ongoing physical health issues, responsive behaviours and/or other neurological disorders. The LEW is responsible for the effective operation and delivery of all recreational programming, which includes assessing, planning, implementing, coordinating, and evaluating a variety of innovative recreational, educational, or therapeutic program activities that will support the achievement of optimal individualized care for the client, and further assists in building their interpersonal relationships and social skills. This position will include both individual and group work.
What You Will Do:
- Assess clients' physical, social, and recreational needs to determine program suitability and develop individualized therapeutic recreation plans.
- Plan, implement, and evaluate therapeutic recreation programs that support clients' physical, mental, and emotional well-being.
- Monitor client progress, maintain accurate documentation, and adjust programs to meet evolving needs.
- Collaborate with interdisciplinary care teams, families, and community partners to ensure client-centered care and support transitions when required.
- Facilitate therapeutic activities including recreation, leisure, fitness, arts, music, and relaxation programs.
- Identify and address potential risks while promoting a safe and supportive environment for clients.
- Support staff, students, and volunteers through training, mentorship, and ongoing professional development.
- Participate in on-call rotations, special projects, and other duties as assigned by Program Manager and/or designated.
- Degree or diploma in Therapeutic Recreation or a related field with a therapeutic recreation focus.
- Minimum 2 years of experience supporting older adults and seniors with mental health, cognitive, neurological, addiction, and/or responsive behavioural challenges.
- Knowledge of and experience with Psychosocial Rehabilitation (PSR) approaches and best practices, including GPA, Montessori Methods for Dementia, PIECES, and Restorative Care.
- First Aid and CPR certification.
- Strong assessment, program planning, facilitation, and problem-solving skills.
- Ability to build positive, supportive relationships with clients and work effectively with diverse populations.
- Excellent communication, documentation, organizational, and computer skills.
- Demonstrated ability to work independently and collaboratively within an interdisciplinary team.
- Reliable, adaptable, and able to manage multiple priorities in a dynamic environment.
- Valid driver's licence and access to a reliable vehicle required.
- Additional language skills are considered an asset.
Work Environment:
- Will be required to work in resident rooms and may be exposed to second hand smoke.
- Required to be on after-hours on call, on a rotating schedule.
- Walk up and down stairs, physically able to assist clients with daily living activities .
- 10%-20% travel to different locations with transit and/or own vehicle, outdoor travel.
- Lifting objects up to 50 lbs.
