Position Summary:
The Clinical Navigator is the initial point of contact for the Outpatient Palliative Care Team. The primary responsibility of this role is to assess patient support needs and to navigate patient care options within Joseph Brant Hospital and within the community.
Responsibilities:
- Acts as the initial contact for all incoming calls from patients, families, community agencies and physicians for the Palliative Care Team.
- Initiates and enters all referrals to the Palliative Care Team.
- Conducts health assessment of patient and determines clinical needs in collaboration with the Palliative Care Team.
- Documents, tracks and prioritizes care requests to ensure all patients and families receive a timely response.
- Completes follow-up calls to patients and/or families to ensure care needs have been met.
- Liaises with CCAC, community agencies, clinics, pharmacies and family physicians.
- Prepares reports and participates in all team meetings and rounds.
- Maintains and updates all patients medical records and files including patient and family risk indicators.
- Drafts all statistical reports as required.
- Uses critical thinking skills to escalate more urgent patient care needs.
- Assists the palliative care team in providing palliative care education to various organizations within the community that provide support toe palliatiive patients and their families.
- Supports the team with respect to research activities and learner placements.
Qualifications:
- Current Certificate of Competence in the Province of Ontario as a Registered Nurse.
- Previous experience working palliative care required, experience in outpatient palliative care preferred.
- Knowledge of pain and symptom management.
- Demonstrated ability to work independently or as part of an interprofessional team.
- Excellent interpersonal and conflict resolution skills.
- Knowledge of support services and resources in the community.
- Excellent written and verbal communication skills.
- Knowledge of Word, Excel and Meditech.
- Demonstrated reliable attendance, punctuality, availability and performance required.
