We are looking for our next leadership addition in the unique role of Care Planner / Community Relations Manager. This role works with families in the community, understanding care options, completing client assessments, building Care Plans and hiring and nurturing care staff. This role also supports community engagement, local brand awareness, business development in the Okanagan region including the key centres of Kelowna, Penticton and Vernon.

Primary Responsibilities as Care Planner

· Conducting client assessments and developing Care Plans, including in-home assessments in the Okanagan region

· Acting as a resource to client families

· Spearheading the Care Team assembly

· Participate in the hiring team and supervision of caregiving staff

· Supervising, managing and motivating the Care Team members

· Relationship development with Caregivers, clients and their families.

· Engage directly with the Right at Home management team, business development representatives, and other staff as an integral part of the company’s goal to continue to be Canada’s leader in the delivery of family-funded home health care services

· Leading team in collaboration with Care Coordinators and Schedulers.

· Assist in achieving an exceptional experience for the Caregivers within our Right at Home system through warm, positive communication, encouragement, inclusiveness and leadership.

· Be an excellent Brand Ambassador for Right at Home

Community Relations

· Participate as part of a Team effort and help foster new clients and growth in the overall business.

· Schedule and conduct meetings with various referral sources in the community to develop business opportunities, expand footprint within the Community.

· Conduct exceptional follow-up to your referral source meetings and meaningfully grow those relationships.

· Monitor and attend relevant community events for community-awareness opportunities.

· Candidate should be confident and comfortable in public speaking and presentations in group settings.

Essential Functions

· Travel across the Okanagan region is an expected and anticipated part of your role. Conducting meetings, consultations, and engagements will happen anywhere within the broad region.

· Participate and support the Team’s growth plans and work closely with Greater Vancouver office

· All time will be spent in the Kelowna office or out in the field for visits (no remote work).

· The Candidate must have a valid driver’s license and access to a personal vehicle.
