Organization Overview
Our Client: Central West Specialized Developmental Services (CWSDS) is a human-services non-profit organization, providing a wide range of residential and non-residential services for individuals with developmental disabilities and complex needs.
PURPOSE OF POSITION
The role assists families with complex situations by helping them to navigate the service systems, bring awareness to community service providers of other community resources, and facilitate multi-agency community case conferences for urgent situations. Your strong knowledge of Halton resources will ensure that families are linked with the most appropriate service to meet their needs. As a Clinical Manager, Service Solutions, you will be required to work in and between systems; fostering relationships between agencies and colleagues in Health, Ministry of Children, Community and Social Services, Education and Youth Justice. Your work will be both at a family-level and system-level requiring the ability to adapt to the environment that you are working in.
This position provides guidance, consultation, and training to service coordinators and community service providers and is responsible for liaising, partner agencies within Halton and Central West Region (Ontario), this includes adult, children and youth agencies, hospitals, schools, and developmental services agencies. The position oversees applications, referrals, supportive living home placements, ensures appropriate resource allocation and support for both children, youth, and adults with complex needs. It includes budget management, approving and tracking invoices/expenditures, and ensuring funding is used effectively within each fiscal year. The role also involves weekly and monthly ministry reporting and committee participation, supporting policy updates and strategic planning.
Duties:
Relationship Building:
o Develop and maintain working relationships with representatives of other organizations including MCCSS, Boards of Education, Hospitals, Developmental Services, Justice to remain update to date on resources and services available.
o Share data across sectors to drive creative system change and build collaborative networks.
o Monitor and track trends, gaps, and pressures within the system to support informed decision-making and strategic planning.
Supervisory Duties:
o Hiring manager for the recruitment and selection of direct reports; work with human resources on all aspects of hiring including job postings, shortlisting, interviews, orientation, and onboarding.
o Manage and monitor scheduling, hours of work, vacation requests, leaves, and performance of direct reports, provide continual coaching and support and follow established processes for addressing the performance of unionized and non-unionized staff. Lead orientation of new staff as required.
Referrals, Placements & Coordination of Services
o Oversee, facilitate, and monitor out of region placements in supportive living homes - including assisting community coordinators and leaders with resource, eligibility criteria, and referral information.
o Repatriate adults, children and youth living in out of region placements, linking with appropriate local community resources, as required.
o Report to and inform the Director, Community Programs of complex cases.
o Have a key role in supporting Urgent Response Forms (URF) from Developmental Services Ontario (DSO) in partnership with the Clinical Manager of Halton Support Services and Director, Community Programs, at CWSDS.
Budget Management & Decisions
o Strong knowledge of creating financial budgets including ability to review and understand large budget breakdowns
o Monitor and administer the program’s annual budget, ensuring the proper allocation of resources, identifying program needs, and factoring in planned changes to service delivery, for each individual supported.
o Ensure all spending is approved, tracked, and properly documented in Excel, a CWSDS specific database as well as file notes for each individual supported.
o Review and endorse short-term, outcome-based funding plans (typically six months) for CWSDS and partner agencies.
o Monitor and communicate regularly to committee(s) on remaining annual budget.
o Ensure surplus money is utilized effectively within service contract parameters and CWSDS finance policies, as required.
o Ensure agencies, people supported and families are aware of funding guidelines and expectations including when specific allocations may not be available in future years.
Ministry Reporting
o Gather, maintain, and create statistical reports and financial forecasts in collaboration with the CWSDS finance department to report to the Ministry on budgets, actual expenditures, funding, unique number of persons supported and infrastructure requirements.
Committees
o Address emerging situations, complex special needs funding approvals and needs of families throughout the year including ranking and prioritizing funding eligibility based on requests and endorsements.
o Review local supportive living home vacancies, facilitate referrals, and endorse service placements with outside paid resources or transfer payment agencies.
o Write outcome letters related to Children and Adult community tables, in a timely fashion and send one for each profile presented.
o Set committee and meeting agendas, coordinate referrals and presentations, ensure proper documentation is prepared and distributed, and support the problem-solving process, with and on behalf of the Chair of the committee(s).
o Work with the Chair to secure final committee approval to update the Policy and Procedure Manual annually.
o Participate in a variety of monthly committees including the Central West Service Resolution Committee and others.
Advice, Training, and Administration
o Act as the subject matter expert, providing resources and advice for complex cases.
o Oversee the placement of independent adult and children in supportive living/group homes, including ensuring insurance, licensing, and service agreements components are up to date. Also, complete other MCCSS reporting pieces like Seriou Occurrence Reporting (SORs) and liaising with 3rd party QAM agency
o Assist with training new employees, as required.
Participate in the on-call rotation for evening and weekend coverage which will include submitting SORs after hours on behalf of outside paid resources as well as for Transfer Payment Agencies (TPAs) who are supporting children in group living situations.
Qualifications:
- Regulated health care professional in Ontario required
- Master's degree in social service, or healthcare (Bachelor’s degree considered)
- 5 - 7 years of progressive management and leadership experience in a health care, social services or development service sector
- Knowledge of developmental disabilities, dual diagnosis, mental health, and complex care needs are highly preferred
- Experience working within Ontario’s Developmental Services, health care, or community mental health systems preferred.
- Ability to interpret and apply relevant legislation, policies, and service guidelines (e.g. MCCSS directives, Health Care Consent Act, PHIPA)
- Experience providing clinical consultation, supervision, or oversight to frontline practitioners in both unionized and non-unionized settings
- Proven ability to manage competing priorities and support staff in high‑pressure environments
- Experience triaging a large volume of applications, coordinating services, and navigating cross‑sector partnerships
- Proven track-record in relationship‑building skills with hospitals, community agencies, justice partners, and developmental service providers
- Excellent communication skills, including the ability to prepare reports, deliver presentations, and facilitate meetings that are emotionally charged
- Strong problem‑solving and decision‑making abilities, particularly in complex or ambiguous situations
- Training in crisis intervention, conflict resolution, or mediation preferred
- CPR/AED "Level C" Certificate, First Aid/CPR Certificate
- Valid G Class Driver's License
