About the Role

We areseekinga compassionate and patient-focusedPart-Time Nurse Case Managerto support patients, healthcare providers, and clinic partners through a specialized Patient Support Program (PSP).

As a key member of the care team, you will help patients navigate their treatment journey by providing education,carecoordination, and ongoing support. You will collaborate with physicians, clinics, pharmacies, and internal stakeholders tofacilitatetimelyaccess to treatment and help deliver an exceptional patient experience.

This is a part-time position requiring approximately2.5 days per week. Scheduled hours must fall within the program's operatinghours ofMonday through Friday, 8:00 a.m. to 8:00 p.m. EST.

What You'll Do

  • Serve as a primary point of contact for patients enrolled in the Patient Support Program.
  • Provide educationregardingprescribed therapies, treatment expectations, medication administration, and adherence strategies.
  • Support patient enrollment and onboarding activities.
  • Coordinate care and communicate with physicians, clinic staff, specialty pharmacies, and other healthcare professionals.
  • Assistpatients in navigating access to treatment and reimbursement processes.
  • Conduct patient assessments and follow-up activities as required by the program.
  • Monitor patient progress and provide ongoing support throughout the treatment journey.
  • Maintainaccurateandtimelydocumentation within program systems.
  • Identifyand escalate clinical or operational concerns asappropriate.
  • Contribute to quality improvement initiatives and program success measures.
  • Build andmaintainpositive working relationships with healthcare providers and clinic partners.

Basic Requirements

  • Active and unrestricted nursing license inAlberta and/or British Columbia(RN, RPN/LPN, or equivalent nursing designation).
  • Diploma, degree, or equivalent combination of education and experience consistent with theP1 Professional Career Framework.
  • Strong verbal and written communication skills.
  • Proficiencyusing Microsoft Office and electronic documentation systems.
  • Ability to work approximately2.5 days per weekwithin program operating hours of8:00 a.m. to 8:00 p.m. EST, Monday through Friday.
  • Ability to travel to clinics and healthcare sites throughout Alberta and British Columbia asrequired.
  • Excellent organizational skills and attention to detail.

Preferred Skills & Experience

  • Experience working within aPatient Support Program (PSP).
  • PriorNurseCase Management experience.
  • Experience supporting patients receiving specialty medications or biologic therapies.
  • Knowledge of reimbursement, patient access, or specialty pharmacy processes.
  • Experience providing patient education and clinical support.
  • Experience collaborating with physicians, specialty clinics, and multidisciplinary healthcare teams.
  • Familiarity with chronic disease management andtreatmentadherence programs.
  • Additionalactive nursing licenses in Canadian provinces.

Travel, Work Environment & Physical Requirements

  • Primarily remote/home-based role with travel asrequired.
  • Travel to clinics, healthcare facilities, and stakeholder meetings throughout Alberta and British Columbia.
  • Valid driver's license and access to reliable transportation may berequiredbased on territory coverage.
  • Must be able to work independently in a remote environment while effectively managing competing priorities.
  • Occasional lifting and transportation of educational or program materials may berequired.
