- Educate patients on health maintenance, disease prevention, and lifestyle modifications.
- Coordinate home care services for eligible patients to ensure continuity of care outside the clinical setting.
- Collaborate with multidisciplinary teams to develop comprehensive treatment plans that prioritize patient safety and satisfaction.
Skills
- Prior hospital experience is preferred, demonstrating ability to manage complex cases in acute care settings.
- Experience in research related to family medicine or primary care is advantageous.
- Proven expertise in managing acute care scenarios, pain management protocols, pediatrics, and patient-centered care.
- Familiarity with medical terminology and proficiency in using electronic health records platforms such as MedAccess and Cerner.
- Background in medical office operations, data collection for quality metrics or research projects, and medication administration procedures.
- Experience providing home-based care services is a plus. Candidates should possess strong communication skills, attention to detail, and a commitment to delivering exceptional patient care within a collaborative healthcare environment.
