Osteoporosis Canada (OC), a registered charity, is the only national organization serving people affected by osteoporosis. OC works to educate, empower, and support individuals and communities on bone health and in the risk-reduction and treatment of osteoporosis.
BACKGROUND:
Osteoporosis is a disease characterized by low bone mass and deterioration of bone tissue. This can cause the skeleton to become fragile, so that even a slight bump or a fall from standing height or less, can lead to a broken bone (fragility fracture).
The goal of the Ontario Osteoporosis Strategy is to reduce morbidity, mortality, and costs from osteoporotic fractures, including hip fractures, using a patient-centered, multidisciplinary approach that is integrated across health care sectors.
The Fracture Screening and Prevention Program (FSPP) is a secondary fracture prevention initiative and a key priority of the Ontario Osteoporosis Strategy. Fracture Prevention Coordinators (FPCs) implement the FSPP in fracture clinics around the province.
JOB SUMMARY:
The purpose of the FSPP is to identify patients aged 50 and over who have a fragility fracture and connect them with support and services to help them reduce the risk of another fracture.
Fracture Prevention Coordinators (FPC) provide osteoporosis screening in an orthopaedic setting. After identifying and assessing men and women who have had a fragility fracture, they facilitate patient access to integrated and appropriate post-fracture care such as bone mineral density testing and/or follow up with an osteoporosis specialist. The FPCs work with orthopaedic surgeons, allied health professionals, fracture clinic staff, and primary care. FPCs also provide information and support to connect these patients with primary care, community-based services and education.
Reports to: Regional Integration Lead (RIL), Ontario Osteoporosis Strategy, Osteoporosis Canada.
QUALIFICATIONS:
Registered Practical Nurse (RPN) or Registered Nurse (RN)
Active registration with the College of Nurses of Ontario (CNO)
Professional Liability Protection Insurance (or willing to purchase)
Other Essential Attributes:
Experience working with Electronic Medical Records (EMR) and computer proficiency, including Microsoft Office, are a must. Excellent oral and written communication skills; ability to work with limited supervision; ability to work as part of a diverse team and the ability to demonstrate sensitivity to the needs of patients.
Fluency in a second language is an asset.
As a condition of employment, you agree to comply with any Health Canada/public health recommendations and/or OC and Hospital policies regarding vaccination, including obtaining a booster shot if recommended. You may be required to provide evidence of compliance, if requested. If you are unable to be vaccinated because of a ground protected under the Ontario Human Rights Code, please indicate this to us.

Salary: $36,000 per annum (3 days/week). Non-negotiable.

- Diligent use of protocol tools to identify and screen fragility fracture patients aged 50 and over;
- Facilitate diagnostic testing for identified patients, as appropriate;
- Communicate with patients’ primary care provider with regard to their patients’ osteoporosis and/or fracture risk;
- Facilitate the referral of eligible patients to an osteoporosis specialist;
- Educate patients about bone health and fracture prevention;
- Refer/link patients to established self-management, fall prevention, and/or education programs, as appropriate;
- Follow up with patients, post-baseline screening;
- Responsible for adhering to privacy and security policies;
- Assist the RIL in providing ongoing support and education to hospital, community, and primary health care staff;
- Identify opportunities for program growth, development, and outreach;
- Participate in regional and provincial team meetings;
- Other duties as assigned.
