Full job description

The Child and Youth Worker/ SIL Support Worker is a member of the interdisciplinary team which assesses, plans, implements and evaluates treatment plans for a given client between the ages of 6-17 years old. Our Supported Independent Living Support Worker would work with youth between 15-26 years of age. The clients are all Indigenous. Employees with this designation perform front-line work in the program and carry the responsibility for the daily care, treatment, nurturance, supervision, health and discipline of the clients. The role is extensive and carries with it responsibilities in the counseling, teaching and training areas, as well as recording and reporting. All potential Candidates must demonstrate high personal competence in the formation and maintenance of caring, therapeutic relationships. The Child and Youth Worker is accountable to the Executive Director.

Job description

  • Ensure that Individual Care Plans are being addressed on a consistent basis.
  • Post person served schedule on Outlook calendar and ensure person served input into their electronic devices. This includes:
  • Appointments (medical, educational, legal, therapy, financial)
  • Meal plan
  • Programming
  • Family Visits
  • Visits at their suite
  • Outings
  • Other relevant items
  • Follows through on specific program goals and objectives needed to ensure agency goals are achieved.
  • Ensures that all person served’ basic needs (food, clothing and shelter) are met as well as their recreational, educational and/or medical needs.
  • Demonstrates good writing, communication, and listening skills.
  • Ensures that all residents’ basic needs, and other needs are met besides food, clothing and shelter.
  • Safely transports resident in own vehicle.
  • Maintains confidentiality of each resident and family.
  • Updates every half hour or each time there is an interaction with person served.
  • Complete any mandatory internal checklists/reports.
  • Complete cleaning checklist.
  • Person served to be supervised at all times.

Qualifications and Education Requirements

  • Diploma in Child and Family Services such as: Social Work, Early Childhood, Community Support Worker, CYCAA Certification or enrolled/attending school in the Child and Family Services area.
  • Demonstrates good writing and communication skills.
  • Ability to travel.
  • Own a smartphone (iPhone, Samsung)

Qualified applicants with Mount Moriah Homes, will be required to meet the following conditions:
