About Us:

Reconnect Health Services is a not-for-profit health service organization in the west end of Toronto. Our services include supports for seniors, caregivers, and people living with mental health and addictions concerns. We pride ourselves on being at the forefront of client-centred care in our sector.

The Mental Health Worker provides on-site assessment, discharge planning and support as well as liaison with existing supports and assistance with goal setting and planning to clients

Responsibilities include, but are not limited to, the following. Other duties may be assigned at the discretion of the Program Manager.

  • Initiates services to clients at the point of intake to ensure that relevant initial service response occurs;
  • Develops respectful and trusting relationships with clients;
  • Supports the service plans and goals in partnership with clients and their supports;
  • Supports the implementation of planned strategies in partnership with clients;
  • Provides a variety of individualized on-site supports including, but not limited to, acsessing medical supports, crisis prevention / intervention, advocacy, etc.;
  • De-escalates unpredictable crisis situations safely and thoughtfully;
  • Liaises and collaborates with other mental health providers, mobile crisis teams, hospitals, physicians, community workers, substance abuse programs, and family members on behalf of clients;
  • Supports and co-facilitates clinical groups for clients, as required;
  • Care coordinates with other resources (within both Reconnect and the community);
  • Delivers all services in a professional manner and according to established guidelines and associated benchmarks;
  • Maintains accurate and complete clinical records.

Technical and Behavioral Competencies Required:

  • Minimum Community College Diploma in social services or related field;
  • Personal lived experience with mental health and or/ addictions
  • Experience with and sound knowledge as it relates to persons with mental health and addiction issues;
  • Familiarly supporting people who were homeless and now housed;
  • At least two years, work experience in mental health (community experience is an asset);
  • Knowledge of mental illness issues, addiction issues, the mental health system and other concepts related to the effects of serious mental illness;
  • Knowledge and training on Naloxone administration, is an asset;
  • Experience and a proven ability to de-escalate crisis situations, assess risk and address issues promptly;
  • Working knowledge of psycho-social rehabilitation and recovery principles;
  • Experience in provision of residential / housing supports to adults with serious mental illness;
  • Excellent skills in mental health assessments;
  • Excellent skills in crisis intervention;
  • Able to work independently and as part of a team;
  • Able to work collaboratively in complex community settings;
  • Excellent communication skills, both oral and written;
  • Familiar with relevant legislation (e.g., Mental Health Act, Personal Health Information Protection Act);
  • Knowledge of housing, health care and community resources;
  • Knowledge of and sensitivity to issues of gender, race, sexual orientation and multicultural issues.

NOTE: Reconnect requires a criminal background check for all employees, volunteers and students.

Workplace Requirements:

  • The HART Hub program is located within a heritage building with multiple levels, which will require staff to navigate stairs throughout their shift.
  • Work hours will vary depending on the needs of clients and the agency.
  • Work scheduled shift(s) assigned
  • Overnight shifts are awake shifts
  • Must be willing and able to work flexible hours.
  • Attendance at special events from time to time may be required.
