Full job description
Blood Ties Four Directions Centre is looking for a Rural Outreach Worker!
Position Overview: Rural Outreach Worker
Note: This role is based in Whitehorse but travels extensively throughout the territory.
The Rural Outreach Worker provides housing-focused outreach and harm reduction services to individuals living outside of Whitehorse, extending supports to rural Yukon communities through a relational and low-barrier approach. As the position is not permanently situated within any one community, relationship-building is a foundational component of the role. Establishing trust requires time, consistency, and respectful engagement, particularly in communities where relationships must be developed thoughtfully and collaboratively.
Supports are tailored to community needs and include housing navigation, harm reduction education and supply distribution, Tenant Wisdom workshops, and systems navigation.
The Rural Outreach Worker collaborates closely with the Rural Educator, the Whitehorse Outreach team, and our Knowledge Keeper to coordinate responses to housing instability across the territory.
Responsibilities include:
- Provide eviction prevention and housing stabilization support
- Conduct housing and support needs assessments for individuals
- Assist clients with housing applications, documentation, and referrals
- Provide tenancy support and follow-up
- Engage with landlords to support tenancy sustainability and resolve concerns
- Deliver housing navigation services and connect clients to community resources
- Support individuals experiencing chronic and complex homelessness
- Coordinate with community partners and service providers
- Maintain accurate documentation within Coordinated Access and HIFIS systems
- Provide early intervention when housing stability is at risk
- Provide harm reduction education related to overdose prevention, safer substance use, and safer disposal practices
- Distribute harm reduction supplies in accordance with organizational protocols throughout rural Yukon Communities, and Whitehorse.
- Support conversations related to guest management, safety planning, and tenancy sustainability
- Promote dignity, respect, and non-judgmental service delivery
- Identify emerging community needs and trends related to substance use and housing stability
- Use harm reduction engagement as a pathway to connect individuals to housing and support services. Harm reduction engagement is considered an integrated component of housing-focused outreach, as these conversations frequently support eviction prevention and tenancy sustainability.
Required Qualifications:
- Willingness to travel throughout rural Yukon
- Experience delivering support services to marginalized populations
- Experience coordinating and delivering health- and wellness-related activities or programs for marginalized populations
- An understanding of the principles of harm reduction and a non-judgmental approach to substance use, with preference given to those with direct experience working in a harm reduction setting
- Experience working with Indigenous people and communities
- Knowledge of cultural safety and trauma-informed care
- Ability to set and maintain boundaries with clients in complex situations
- Post-secondary education in the social, human, or health services field or equivalent in experience
- Current Standard First Aid and CPR
- A valid driver’s license with a clear driver's abstract
