Manager, Integrated Care Operations
Temporary Full-Time (approximately 12 months)
Pay Rate: $115,156 - $137,503 Annually
The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, and Transitional Care. The care and services provided at TGHC help facilitate patient flow through the health care system and ensure recovery for those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church & Bloor St), Harbour Lights, and a Specialized Care Centre (Victoria Park & McNicoll. We are seeking a collaborative professional for a temporary full-time opportunity to contribute to our purpose and values – the Manager, Integrated Care Operations - who is passionate about making a meaningful difference in the lives of patients and families through integrated outpatient health care services. At Toronto Grace, we live our values in all that we do and value our dedicated staff, volunteers, and physicians.
The outpatient clinic serves 10-15,000 visits per year and currently encompasses rheumatology, respirology, geriatrics, penicillin allergy testing, psychotherapy, and spasticity, and is served by 15 physicians. This position is located at our 650 Church Street site and is a temporary full-time position for a period of approximately 12 months.
In the coming year, we anticipate expanding to a satellite site in addition to our main site at 650 Church Street, Toronto.
Position Summary:
Reporting to the Chief of Medicine, the Manager, Integrated Care Operations provides leadership and expertise for outpatient, clinical program and diagnostic imaging services, specifically Ultrasound, Mobile X-Ray, and Bone Mineral Density testing. They collaborate with Respiratory Therapy, including Pulmonary Function testing. The Manager, Integrated Care Operations plays a critical role in the oversight of specialty clinic visits, diagnostic imaging, and integration of virtual programming. This role is to map and improve patient journeys between inpatient, outpatient, DI, and virtual rehabilitation, including identifying and implementing processes that reduce bottlenecks and duplication.
In addition to operational and clinical oversight of integrated clinical services, this Manager collaborates with physicians, managers, healthcare providers, administrative and systems staff and external partners towards achieving the goal of patients in one of our programs being able to access the excellent care we provide in another domain.

Responsibilities:
- Responsible for the management of day-to-day operations and activities, assigning work and ensuring flow for office operations, administrative & diagnostic services, such as ultrasound, mobile x-ray, and bone mineral density
- Collaborates with Remote Care Monitoring Program & Virtual programming components
- Identifies, integrates, and encourages opportunities for linking patients to virtual care for seamless services
- Forecasts, plans, and coordinates effective workflow in functional areas such as reception, appointment bookings, billing systems, and quality improvement processes for the QGFDSA department
- Initiates and maintains positive and professional relationships with administrative, clinical and physician staff of the unit, gathering feedback and implementing processes that are effective and efficient, attentive to quality improvement and patient-centred care
- Financial management - prepares department budgets and monitors ongoing expenditures to ensure compliance, seeking processes and systems that contain costs and ensure quality patient services
- Accountable for gathering, maintaining, and reporting statistics regarding outpatient registration, appointment bookings, and smooth-running integration of patient care from an outpatient environment to an inpatient setting
- In collaboration with Human Resources / Labour Relations recruits, orients, evaluates and addresses performance and discipline as appropriate for staff. Establishes appropriate levels of staff to support operational objectives
- Ensures planned maintenance for diagnostic equipment and provides support in the event of machine or equipment failure during typical operations
- Maintains accurate and comprehensive documentation of health records and statistical data
- Maintains and respects confidentiality of patient health records and information
- Collaborates with employees, physicians / managers, and human resources to promote a culture of safety, wellbeing, and continuous improvement across all sites
- Participates in Workplace Safety Inspections and in WHMIS Program as required
- Creates a safe and supportive working environment for all
- Minimum 5 years of progressive leadership experience in health or community service organizations, preferably within an interprofessional or primary care setting
- Recent, relevant, managerial experience in a health care facility, imaging service, or clinic with diagnostic imaging services such as cardiorespiratory, cardiovascular, or obstetrics/gynecology is preferred
- Demonstrated success in interprofessional collaboration, partnership development, or integrated care models
- Exceptional interpersonal, communication (written and verbal), and facilitation skills with the ability to engage and collaborate with diverse internal and external stakeholders expected
- Proven project management skills to identify an area for improvement and execute implementation such as assessing and redesigning physical space, supply management, and procedures, etc.
- Proficient analytical and engagement skills coupled with an established record of applying adult education principles to promote learning and quality improvement required
- Coupled with the above required leadership skill set with managing a multidisciplinary clinic, the ideal candidate will be an R.N., Registered Technologist, or Allied Health professional with successful education from a recognized university and professional designation/registration in good standing with their professional college/association
- Well-developed time management and prioritization skills are an expectation.
- Must practice sound judgement and exercise high emotional intelligence, with the demonstrated ability to build trusting relationships with all levels of stakeholders
- Working experience with computerized diagnostic and scheduling systems strongly valued, specifically, Avaros/OSCAR, CHIME, RingCentral, SRFax, Velox & Cliniconex
- Ability to work both independently and as a team member with a proven customer service and solution-focused approach
- Strong work ethic and good attendance record required
- Demonstrated knowledge of relevant legislation: Human Rights Code, Occupational Health & Safety Act, Personal Health Information Protection Act (PHIPA), etc.
- Ability to travel to satellite sites and external partner sites as required
