OPPORTUNITY

We are hiring a Community Services Facilitator to join our team. This full-time position is an opportunity to put your knowledge and technical skills to work on a variety of member focused tasks.

Reporting to the Manager of Strategic Initiatives and Community Services, the position of Community Service Facilitator acts as a first point of contact for all outside correspondence for individuals, families and community stake holders regarding services and programs provided by VITA or the Developmental Services Sector.

The focus of the position is to gather all pertinent information necessary to access VITA’s programs and services, organize access to VITA service, identify and filling funded and FFS service. In addition, maintain records and advocate member service needs to the DSO and or where applicable at other sector groups.

This position complies with all VITA Policies and Procedures, follows VITA’s Mission, Vision, and Value Statements. It is agreed that this position will work in compliance with the provisions of the Occupational Health and Safety Act and Regulations and with the Ministry of Community and Social Services. THE ROLE:

  • Maintain member database (PCC) Ensure all members information is up to date for both those wishing to access Vita's services and for members receiving VITA services.
  • Create and maintain effective working relationships with members, families, and other support service agencies.
  • Navigate local and sector access mechanisms.
  • Act as a service support for the public, prospective members and their families by providing service information about VITA and community partners.
  • Complete and manage the intake process, including waitlists for all members interested in VITA services and support.
  • Advocate for members awaiting VITA services and relay all up to date and pertinent member info to the DSO.
  • Maintain statistical data and prepare reports related to services offered by VITA and for future service opportunities.
  • When applicable, manager and coordinate Safe bed requests coming from the designated community partner leads in both Toronto and CR.
  • Develop and coordinate initial service agreements for VITA members accessing service.
  • Provide leadership with various funding initiatives that require responsiveness on behalf of VITA services and support.
  • Liaise with local stakeholders, community support agencies as needed to provide more community opportunities for members and potential members.
  • Participate in Community & DS Sector Committees / events representing VITA.
  • Work closely with staff to support member service goals.
  • Collaborate with staff, members, families, caregivers, and external agencies by participating in and/or leading meetings related to members’ changing needs.
  • Support staff with challenging situations which involve members and/or their families.
  • Perform other related duties assigned by the Supervisor, Manager, Director, or delegate.

WHAT YOU BRING:

  • Post-Secondary Education in Social Services (DSW, SSW, B.SW, B.A )
  • 3-5 years' experience working in the developmental services sector
  • Experience coordinating member service within a Public Sector environment
  • Strong and effective communication skills particularly surrounding family / caregiver support an asset
  • Ability to travel within Toronto/York Region and work at different service locations based on member and service needs.
  • Flexible availability, including weekends and evenings. Work hours may vary based on service and member needs; the position is full‑time at 40 hours per week (8 hours per day).

NICE TO HAVE:

  • Experience in a Non-Profit Organization.
  • Experience and ability to work with individual with disabilities.
  • Valid driver’s license.
