Full job description

The Quality Improvement Specialist will champion quality improvement in team-based care though data analysis, critical thinking and applying improvement science. The incumbent will be responsible for supporting two FHT's, North Peel FHT and Halton Hills FHT in the implementation of data quality initiatives, improving the flow and use of information, developing queries and analytical products that support leaders in their quality improvement goals and teams engaged in clinical process change. The incumbent will work collaboratively with health care practitioners to advance the quality agenda with a focus on improving access, patient centered care, and effective evidence-based practice. The incumbent will also be expected to participate in broader data harmonization initiatives and champion the use of data and performance analytics within and across the OHT.

The candidate will also support advancing QI planning and implementation among other primary care networks or individual practices within the Ontario Health Team. A schedule will be established on time spent with each of the lead organizations and other primary care practices as identified., and the incumbent will take direction from both Executive Directors for assigned work. The candidate will be expected to work in person and will need to travel to each of the sites as required.

Responsibilities

  • Establish and implement a quality-focused plan that supports the FHTs in demonstrating effective practices, outcomes and value
  • Be a subject matter expert in data integrity and lead the analysis of data, using statistical methodologies, to engage leaders and health care providers in discussions grounded in data and evidence that lead to corrective and preventative actions
  • Establish and evaluate metrics and indicators to measure and monitor performance
  • Design, test and deploy tools and processes for extracting and managing performance data
  • Identify gaps in data availability, and define and implement solutions to close the gaps
  • Lead the planning, development, implementation, and evaluation of quality improvement plans
  • Provide consultative and analytical support regarding the collection, interpretation, analysis and presentation of data to help teams ensure that clinical information management practices support best practice standards.
  • Be a consultant and quality improvement coach on data management and performance measurement, building quality improvement capacity across the teams
  • Create effective working relationships with the teams and manage data quality, business process improvement, report development and other projects using effective project management practices
  • Support the FHTs to meet their reporting mandate by providing accurate and detailed data and information
  • Work with your peers across the province on standards development and data harmonization priorities
  • Conduct analysis and develop reports and presentations using statistical reporting packages, and present this information to a variety of audiences
  • Provide leadership in knowledge dissemination activities

Qualifications

  • A university degree, Master’s preferred, in a related discipline: Business Administration, Statistics, Health Sciences, Health Informatics or Information Management
  • Extensive training and work experience in Quality Improvement
  • 3-5 years of related experience in a health care environment
  • Expertise in quality improvement methodology
  • Demonstrated expertise in data quality and clinical information management best practices
  • Knowledge of primary care and demonstrated interest in the use of data for quality improvement
  • Understanding of requirements for working with personal health information and safeguarding data
  • Strong applied knowledge of one or more EMR/EHR systems currently in use in Ontario, preference given for knowledge of Practice Solutions.
  • Knowledge of databases and business intelligence tools/report design, in particular strong proficiency in Excel and Access; IBM Cognos, Jasper Reports, Hummingbird BI, SQL, would be an advantage.
  • Sound knowledge of performance improvement techniques and practices used in healthcare settings, process redesign and system implementation experience
  • Valid Ontario driver's license and access to a vehicle
  • Proven ability to develop and maintain strong relationships with partners
  • Highly effective communication, presentation, and interpersonal skills
  • Ability to identify complex problems and reviewing related information to develop and evaluate options and implement solutions with all types of health data.
  • Strong report writing skills
  • Ability to handle a variety of concurrent assignments and to work independently
  • Detail-oriented with quantitative analysis and problem-solving skills.
  • See value and able to work effectively as a collaborative team member
  • Demonstrated leadership skills, critical thinking and is proactive in their work
  • Exceptional organizational and time management skills
  • Experience teaching, facilitating groups and knowledge of adult learning principles
  • Proven skills in developing and implementing evaluation tools
  • Experience working with confidential and time sensitive documents and information

Pay: $74,000.00-$84,000.00 per year
