Full job description
Peak House is a voluntary, ten-week, live-in program for youth experiencing problems with drugs, including alcohol. We operate within the harm reduction spectrum with a trauma informed, client centerd, and social justice framework.
Established in 1988 by Pacific Youth and Family Services Society, the program serves youth from across British Columbia who, due to the serious nature of their substance misuse, require a more intensive level of service than can be provided in their home community.
Intake Coordinator
Overview:
The primary role of the Intake Coordinator is to support youth during pre-intake, intake, and transitioning back into community post discharge. This position may require some travel (company vehicle) within the Lower Mainland. This position is often the first point of contact between community and the organization and requires a high degree of diplomacy, warmth, compassion, and professional communication.
This position reports to the Program Manager.
Full Time – 35 hours/week. Salaried wage starting at $30.00
Responsibilities:
- Responsible for all pre-intake communications with prospective clients, their caregivers and supports.
- Required to conduct a pre intake meeting prior to intake to include a site tour, Q&A, and clarification of guidelines and program expectations
- Responsible for program promotion on and offsite
- Responsible for maintaining a minimum 85% bed occupancy rate
- Attending weekly screening committee conference call for new referrals
- Attending community networking meetings
- Liaising with community resources and supports
- Facilitates referral review at weekly consultation meeting
- Maintains excellent knowledge of community resources (across BC) relevant to client population
- Attends both weekly and monthly staff meetings & staff trainings
- Liaise with nurse practitioner around ensuring medical needs are met (prescriptions, bloodwork/testing, appointments, etc.)
- Liaise with pharmacy to secure medication with support from the nurse practitioner as needed
- Liaise with school program teacher
- Responsible for out of house transportation coordination to and from the program as needed.
- Responsible for meeting youth off-site for transition support where appropriate
- Completing quarterly and semi-annual statistical reports
- Participates in and contributes to a positive group culture
- Other duties as directed by Program Manager or their designate
Knowledge, Skills and Abilities:
- Degree in related discipline, work experience in the mental health and substance use field - or a combination of both will be considered
- Minimum of three years experience in a live-in setting with youth
- Minimum of three years experience with substance related issues (preferably with youth)
- Knowledge of community resources across British Columbia
- Excellent written and oral communication skills
- Excellent computer and technology skills including basic word programs, internet, social media platforms , AI, and statistical database experience.
- Experience in youth and family counselling
- Excellent organizational and time management skills
- Diplomacy, and the ability to work within systems that can be challenging
- Valid Class 5 B.C. Drivers License
- Comply with the requirements of a criminal record check prior to commencing employment and every five years thereafter
- Emergency first-aid and CPRA certification
- Negative TB test prior to commencing employment
