Overview
The Community Support Coordinator, TIFS (Try it on for Size), will be responsible for the design, implementation and development of life skills related to living semi-independently through practice of hands-on tasks and theoretical experiences, providing instruction, coaching and evaluation to support and assist individuals in developing and learning skills required to live semi independently. Connecting with all stakeholders throughout, they will create a one-year plan and schedule, with ongoing assessment and evaluation to ensure that each individual is practicing those skills, progressing well and moving towards living semi-independently.
Responsibilities
Support & Services
•Adheres to standards of practice and code of ethics
•Develops a meaningful relationship and partners with individuals, their families, formal and informal supports and teams to provide integration of skills to lead to successful independent living in their community
•Contributes to the development of best practices unique to independent living and skills required to living on their own
•Collaborate with, assist and support individual receiving support (and their families) to set realistic goals, determine interests, abilities, needs and potential barriers to living independently and to build skills and readiness in preparation for living independently. Secure and maintain access to community activities and programs.
•Provides leadership and facilitation of skill building, independent living readiness, community development and person-directed practices.
•Advocates and develops relationships with key stakeholders to ensure that individuals are equally valued members of their community
•Provides support to the process for fee for service dollars, from consultations to service delivery.
•Assists in person directed planning and advocacy, assessing individuals’ strengths and needs, determining natural existing supports as well as availability and appropriateness of community and professional supports as needed and relevant to program. Facilitate development of goals, skills, and Person Directed Plans.
Training and Instruction
•Utilizes a variety of instruction, coaching and training methods to engage individuals in a variety of learning opportunities conducive to independent living
•Provides training through small groups and 1:1 workshops that promote independence using a variety of methods. Ability to adapt style based on needs of individual or group; support the development of networks of support for individual and family
•Provide one-on-one coaching, and skill building in the area of living independently with support
Recruitment and Intake
•Promote the TIFS Toronto program to individuals, families, community partners. Develop and maintain partnerships within the community.
•Collect and complete all relevant documentation for intake into program
•Self assessment & Goal Development: in partnership with families and individuals living at home, facilitate self readiness for independent living, develop goals/plans and identify support needs
•Ongoing review and development for all tools for skill development throughout program participation
•Provide liaison/consultation on behalf of individuals/families for CLTO, MCCSS and community programs; providing support to the process for individualized dollars for consultations to service delivery
•Program development, creation of specific learning scenarios to correlate individuals’ abilities and goals to increase independence skills, researching community supports pertaining to individuals’ goals and program
•Maintain all documentation, completion of reports as required, maintaining open communication with individuals, family, community partners, all stakeholders
•Provide ongoing leadership and guidance in the area of skill building, independent living readiness, working with families, and community support.
Experience
University Degree or minimum two years College Diploma with professional experience preferred.
2-3 years related experience in instruction, workshops, facilitation, skill building, instruction, community development, working with families, advocacy and networking. Staff should be comfortable with facilitating small groups, and instructing them on specific tasks required to live independently. Use of car an asset.
Ability to problem solve and adapt program requirements based on needs.
Proficiency with computers using MS Office (Excel, Word, Outlook), and other software programs needed to produce documents, reports, databases, presentations and communication vehicles for staff training and development.
