LPN Home Care Case Manager

Home Instead® is looking for a energetic, dynamic, competitive and sales oriented LPN to join our team as a Case Manager.

If you've had Home Care experience and are ready to use you skills in a real leadership role by guiding families through the "Home Care Maze" this might be for you. You will be relied upon to by our clients to help navigate the retirement and care options they are facing.

At Home Instead our LPN Case Managers work collaboratively with families who are at the beginning of their care journey and are looking for care solutions for their loved ones. The LPN Home Instead Case Managers must have advanced communication skills along with practical seniors care experience to support this important role. Our LPN Case Managers will be required to guide our often uncertain and stressed families.

Those LPN's who have worked in Facilities are welcome to apply if they have community home care experience.

To be successful the candidate must:

  • Have a valid LPN License
  • Have a drivers license with vehicle
  • Be extremely articulate with Perfect Command of the English language.
  • Have at least 2+ years of Home Care Experience
  • Thrive in a results driven culture
  • Be able to multi-task with a busy work pace
  • Have confidence when communicating
  • 3 years Sales experience required

This position involves speaking with family members who are searching for answers to their senior loved one’s care needs. This means you will interact with a variety of different people who are at different stages of their care journey, confident and strategic communication skills are required.

You will need to use influencing skills to explain to families why they should choose Home Instead for their loved ones.  This role is responsible for effectively managing multiple client demands that come with their changing needs.  This individual must be able to work in a fast-paced organization and have excellent communication skills, be service-oriented and have the ability to work well in a team environment. Knowledge of health care will help you understand the unique challenges our clients face. You will be busy and we are looking for that special person who loves hard work! This is how we do Home Care! No 2 days will look alike as you will be visiting clients and their families throughout the city!

The LPN Case Manager is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/Care Giver introductions and quality assurance visits with existing clients. They use our unique consultative sales approach to determine each individual client’s needs to provide solutions and create a tailored service plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours. T_he LPN Case Manager will often speak with families who are unsure of which pathway is the “best” so our Manager must have a confident personality with the ability to communicate professionally & effectively._

This leadership position will include teaching and training our unregulated workforce. This will include classroom training, as well as in home visits to train and support our amazing Care Professionals.

This unique role will have you working Monday to Fridays, and working 1 in 5 weekends, with one night of call per week.

Qualifications:

  • Must have excellent written and verbal English communication skills
  • Direct experience working with Seniors and their families
  • Experience with sales and customer service
  • Supervisory experience is an asset
