HIV Registered Nurse – Sanctum Care Group

About Sanctum

Sanctum is Saskatchewan’s first and only HIV Hospice and Transitional Care Home. We provide 24/7 transitional care for people living with HIV who face complex social, medical, and high‑risk conditions—such as homelessness, substance use, and trauma—that make community-based disease management challenging. Sanctum also offers compassionate end‑of‑life (palliative) care for individuals living with HIV who have a palliative diagnosis.

Our care model is rooted in harm reduction, trauma‑informed practice, and person‑centered support. The Registered Nurse plays a critical leadership role in upholding these values and ensuring high‑quality, holistic care for every client.

Position Summary

The Registered Nurse (RN) is responsible for providing clinical leadership, direct nursing care, and collaborative support within Sanctum’s interdisciplinary team. This role ensures that client care is safe, evidence‑based, trauma‑informed, and aligned with Sanctum’s mission and policies. The RN supports home support staff, advocates for clients, and maintains clinical excellence in all aspects of transitional and palliative care.

Key Responsibilities:

Clinical Leadership & Team Support

• Lead, guide, and support home support staff in delivering high‑quality client care.
• Communicate care plans clearly and provide ongoing feedback and coaching to staff.
• Address day‑to‑day concerns raised by clients or staff, ensuring timely and appropriate resolution.
• Model trauma‑informed, harm‑reduction, person‑centered care in all interactions.
• Navigate difficult conversations with staff or clients when incidents, conflict, negativity, or toxicity arise.
• Uphold and enforce Sanctum Care Group (SCG) policies and procedures; escalate concerns through appropriate communication channels (Supervisor, OD, PD, ED).

Client Care & Clinical Duties

• Create, update, and maintain individualized client care plans and weekly care summaries.
• Administer medications, verify medication accuracy, and complete weekly medication checks.
• Ensure Medication Administration Records (MAR) are accurate, current, and clearly documented.
• Provide wound care, symptom management, and other nursing interventions within RN scope.
• Conduct critical conversations with clients regarding goals, expectations, and adherence to house rules.
• Admit new clients, ensuring a supportive and trauma‑informed transition into the program.
• Coordinate appointments and collaborate with external healthcare providers to support client needs.

Collaboration with Physicians & Clinical Partners

• Participate in daily physician rounds and advocate for client needs.
• Work closely with infectious disease physicians; prepare all required documentation and information for clinic days.
• Arrange immunization clinics with public health.
• Communicate effectively with interdisciplinary team members to ensure continuity of care.

Palliative Care

• Provide compassionate end‑of‑life care to clients with palliative diagnoses.
• Participate in the palliative care RN rotation to ensure 24/7 coverage when required.
• Program & Organizational Support
• Participate in the Manager On‑Call rotation (one week of 24/7 on‑call every 6–8 weeks).
• Review and discuss referrals with the Program Director and Executive Director.
• Assist in reviewing, developing, and updating policies, procedures, and work standards.
• Maintain high standards of documentation, communication, and clinical practice.

Qualifications

• Registered Nurse Degree (BScN or equivalent).
• Current and active RN license in Saskatchewan.
• Knowledge and experience in HIV care and addiction medicine.
• Training or experience in harm reduction and trauma‑informed care (asset).
• Ability to work as the sole RN on site and confidently manage situations within RN scope.
• Strong organizational, communication, and leadership skills.
• Ability to work effectively with individuals experiencing homelessness, substance use, trauma, and complex social needs.

Pay: $42.00-$49.00 per hour
