Xa’xtsa (Douglas) First Nation is seeking a compassionate, community-minded Community Health Nurse who wants more than just a job; someone who wants to build relationships, strengthen wellness, and be part of something meaningful.

The Opportunity

As our Community Health Nurse, you will work closely with the Health Lead to provide culturally safe nursing care across the lifespan; from newborns to Elders. You’ll deliver hands-on clinical services, provide health education, support chronic disease management, and help promote overall wellness throughout the community.

This is a role where relationships matter. Where you are not just treating patients; you are supporting families, walking alongside community members, and contributing to long-term health and wellbeing.

Job Summary
We are seeking a compassionate and dedicated Community Nurse to provide essential healthcare services within the community. The ideal candidate will deliver patient-centered care, promote health and wellness, and support individuals in managing their health conditions. This role offers an opportunity to make a meaningful difference in patients' lives through personalized nursing care in diverse settings.

Responsibilities

  • Conduct home visits and community-based assessments to evaluate patients' health status
  • Educate patients and their families on health management, disease prevention, and wellness strategies
  • Monitor patient progress and document care activities accurately in EMR
  • Collaborate with multidisciplinary teams to coordinate care plans and ensure continuity of care
  • Assist with health screenings, immunizations, and health promotion programs within the community
  • Respond to patient inquiries and provide guidance on managing chronic conditions or acute illnesses

Skills

  • Proven experience in patient care and nursing practices
  • Strong understanding of medical terminology and clinical procedures
  • Excellent communication skills to effectively educate and support patients and families
  • Ability to work independently while maintaining a high standard of professionalism
  • Compassionate attitude with a genuine desire to improve community health outcomes
  • Organizational skills to manage multiple patient visits efficiently
