Position Title: BILINGUAL HEALTH PROMOTER
Department: Health Promotion and Community Services
Location: Various locations
Status: One Year Leave Coverage, Bargaining Unit position
Hours/Shifts: 35 hours per week
Schedules/ work location are subject to change to meet operational needs.
Job Summary
Black Creek Community Health Centre (BCCHC) is a community based non-profit organization that provides primary health care and health promotion programs and services to community residents. Reporting to the Manager of Health Promotion and Community Services, the Bilingual Health Promoter will be responsible to develop, co-ordinate, implement, and evaluate the health promotion programs. The Bilingual Health Promoter will facilitate the integration of a health promotion focus in Centre activities, operating within the values and principles of the Organization.
Key Responsibilities
· Support the mission, vision and values of BCCHC.
· Support the coordination, design, implementation, and evaluation of health promotion programs and initiatives.
· Develop partnerships and prepare grant applications to support new and ongoing programs and initiatives related to health promotion needs.
· Design and carries out needs assessments, focus groups, surveys and program evaluations in collaboration with staff, etc.
· Provide leadership and expertise on various program priority areas, including participation in joint programming as required (e.g. Community Health Ambassadors, Harm Reduction, Community Engagement).
· Develop and deliver health education materials and programs to increase knowledge and awareness.
· Collaborate with community partners and staff members to: identify service/information/health promotion gaps in the community; and develop, implement, monitor and evaluate strategies, services and programs to meet identified needs.
· Facilitate / co-facilitate groups and community initiatives.
· Assist in the development of projects and initiatives that help clients and community to increase control over and to improve their health.
· Act as a resource to task groups and committees on issues related to the BCCHC’s priority populations.
· Prepare reports, displays, and make recommendations related to health promotion as required by other Agency staff, the Manager and the Director.
· Participate in organization-wide activities and committees, as required
· Handle other duties as assigned
Required Qualifications:
· A Master's degree in Health Promotion or Social Sciences or related area, or an equivalent combination of a baccalaureate degree and extensive experience relevant to the primary duties of the position
· 3 to 5 years of recent relevant experience, including:
o working in a marginalized/racialized community setting and with diverse and intergenerational groups
o community outreach and networking with stakeholders
o experience leading program planning including needs assessments, logic model development and evaluation, social marketing, research, advocacy and policy development
o experience in building, contributing to and sustaining effective community partnerships, networks, coalitions and inter-agency teams and building community capacity
o health promotion and education/awareness campaign
· Excellent knowledge of community development models and program evaluation using an anti-racist, anti-oppression approach.
· Excellent facilitation, team building and conflict management skills as they apply to interdisciplinary teams.
· Knowledge of community resources and services.
· Knowledge of People-Centered Healthcare and the Community Health Centre model of care.
· Proficiency in the use of computers and various software applications.
· Experience in the use of computers and various software and EMR applications
· Strong facilitation skills that will foster, promote and advance health equity and social justice within groups and community initiatives.
· Strong organizational and time management skills.
· Ability to work flexible schedule including evenings and weekends.
· Fluency and proficiency in written and oral French. (Advance Minus Proficiency Required)

Compensation: $67,732.72 – 77,143.58
