Career Opportunity
We are looking to hire one Complex Support Coordinator to join our Community Networks of Specialized Care team! In this position, you will provide short-term goal-oriented, person- and family-centered support coordination for people with developmental delays and/or dual diagnosis who are experiencing complex needs and challenges involving more than one sector (e.g. health care, education, housing, justice, mental health and substance use). The work is focused on supporting individuals and families through periods of difficulty or crisis, and enhancing capacity across the Developmental Services (DS) sector. The position also plays a key role in supporting other service providers through coaching, education, and collaborative planning.
RESPONSIBILITIES:
- Provide complex support coordination in alignment with the CNSC model using a person- and family-centred, recovery-oriented, and trauma-informed approach
- Support cross-sector coordination support (e.g. assessment, clinical planning, housing, planning tables) to maximize existing resources and services effectiveness and resource use
- Develop individual profiles and present them at local and regional planning tables to support access to services, specialized supports, and short-term funding (e.g., Urgent Response submissions to Developmental Services Ontario (DSO)).
- Write profiles for the person receiving services and present to local and regional service tables (e.g. Halton Service Solutions; Peel Adult Service Solutions, etc.)
- Collaborate with key partners such as the Health Care Facilitator Nurse, Dual Diagnosis Justice Coordinator, and Service Solutions Facilitator in the region the person supported lives and also across regions
- Attend the provincial CNSC committee for Complex Support Coordinators to stay informed of current practices and system changes
- Facilitate applications, case conferences and other meetings in order to get required supports and services (e.g. specialized services, group living, etc.). This would also include clinical supports, psychiatric support, psychological support and specialized accommodations
- Support families, caregivers, and supportive living environments with coordination, advocacy, and access to supports as well as in building capacity through education and resource sharing
- Liaising with sector partners, including those in the health care system, mental health and justice
- Monitor and manage active and inactive files, ensuring goals are reviewed and updated and that transitions to other supports occur as appropriate. Developing appropriate strategies for people who may be resistant or distrustful of services and who are at risk in the community.
- Participate in regional initiatives, working groups, and community planning bodies, as requested by your supervisor.
- Supporting the Vision, Purpose and Vales of CWSDS and the CNSC
- Ensure compliance with CWSDS policies and procedures
- Maintain timely and accurate documentation, data entry, and statistical reporting.
- This position serves the Central West Region (Waterloo, Wellington, Dufferin, Halton, and Peel), working collaboratively with local and regional partners to navigate systems, coordinate cross-sector planning, and access appropriate services and resources. Travel across regions and locations supported by CWSDS is required.
- Perform other related duties as assigned
WHAT QUALIFICATIONS AND SKILLS DO YOU BRING?
- Completion of a formal university degree in Social Services at the undergraduate or post graduate levels. A combination of various work and education experience will also be considered.
- Minimum of 5 years of experience working with individuals with developmental disabilities and/or dual diagnosis, using a person-centered, recovery-oriented approach.
- Knowledge of Ontario’s developmental services system, and experience navigating mental health and substance use, justice, and housing sectors.
- Demonstrated ability to build consensus, resolve issues, and develop plans for individuals with complex and multiple needs. Strong ability to network, and develop plans for individuals with complex and multiple needs.

- Excellent written, verbal and interpersonal communication skills.
- Thorough understanding of the bio-psychosocial factors contributing to developmental disability and mental illness.
- Positive attitude and ability to work collaboratively with formal and informal cross-sector community services/resources and family systems.
- Sound judgement and ability to manage crisis and high-risk situations.
- Well-developed risk assessment, problem-solving and advocacy skills.
- Ability to work independently with minimal direct supervision.
- A satisfactory Police Records Check (Vulnerable Sector Screening).
- A valid G Ontario driver’s license and vehicle to travel throughout the region.
